Comments:_1/23 first day of clinical. Still orienting to unit, policy and procedures. Had NSTEMI pt. Continue to work on pre planning
to understand pertinent labs, medications and nursing care to the patient. Be sure to understand medications on how it will impact
the patient, why patient is receiving and how to administer safely to the patient. Heparin drip is a high alert med that needs special

attention in regard to PTT, platelets and side effects. Did well with caring for patient and helping staff and other students. 2/6 –
Improved pre planning. Cont. to work on SBAR to be concise and efficient. Cont. to link up labs, diagnostics, meds and care to pt. Be
sure to include the Lasix drip with SBAR with mg/hr since it is a large part of the care for the pt. Cont. to think if the foley is still
pertinent for the pt. In this patient’s case, yes for strict I&O. You can add that to your SBAR and assessment. See how diagnosis of
pancreatitis lead us down different routes - ? appendix involvement, cardiac issues rhythm and heart failure, increase INR– patients
are like a box of chocolate – never know what you are going to get! Good job with assessments and seeing the changes in the
patient. Worked well with staff and pt. Cont. to practice skills like IV pump, drawing up meds, giving meds, med pass and other skills
in open lab. 2/20 – Had pt with A fib and RVR – look for anticoagulation see patho on concept map – link why Mag and K+ are
important on a pt like this and why thyroid was checked. Link labs and meds to your pt. Able to see TEE and cardioversion. Cont to
work on SBAR for being concise and efficient with pertinent information. Pt with AFib with RVR on cardizem drip at 5 mg/hr. Previous
history of ablation at UVA 2010. For TEE/ cardioversion today. BNP 6216, troponin neg. Participated in post mortem care today.
_3/13 – progressing well week to week. Good pick up with GI bleed and no occult blood. Good thinking and pulling to bedside. Good
organization of the day and assisting peers and staff. 3/27 – Pt with complete care. Be sure to check lines and drains with initial
assessment. Foley bags on same side as stat locks to avoid crossing over body. Assisted in transferring pt into air flow bed.
Remember the basics with caring for these people, turning repositioning, mouth care, ROM, etc. Lasix drip watch creatinine levels,
K+ levels, urine output and weights. 4/10- - Good end to the semester. Slow day on unit. Did assist with other duties.. Be sure to
review summary page if at RMH. It was a pleasure to have you in clinical.
