AFFIDAVIT

I, JUANITO B. MAGBANUA JR., of legal age, single, and a resident

of Hda. Adriana #2 E Lopez , Silay City, Negros Occidental, after having
been duly sworn to in accordance with law, do hereby depose and state the
following:

1. That I am a voluntary member of the Social Security Service (SSS)

with UMID ID No. CRN 0007-1780808-7;

2. That I am self-employed as I have no employer other than myself

and derive income from my physical and mental efforts in the
business of marketing, buying and selling of various merchandise or
commodities from November 2017 until the present;

3. That my monthly income ranges from 3,000 pesos to 4, 000 pesos;

4. That I intend to avail of the SSS Education Loan Program for the
college education of my younger brother JAKE B. MAGBANUA;

5. That I am executing this affidavit to attest to the truth of the

foregoing and for whatever legal purposes that his statement may
serve.
