TOPIC AND RATIONALE

This syndicated seminar focuses on the topics of compassion fatigue and

burnout in healthcare providers, which include nurses, physicians, and other

healthcare workers. ‘Compassion fatigue’ is described as the phenomenon of

desensitization of healthcare providers (HCPs) towards patients, resulting in a

subsequent loss of his/her ability to adequately care for patients (Hinderer et al., 2014).

Compassion fatigue in HCPs is caused in large part due to ‘burnout’, which is

experienced by HCPs from feelings of frustration and powerlessness (El-bar, Levy,

Wald, & Biderman 2013) stemming from several factors of their occupation. These

factors include unhealthy work environments, high workload, poor staffing, and

increasing complexity of care without adequate resources.

The Philippines is one of the world’s leading exporters of registered nurses

(RNs) and yet suffers from low job satisfaction among nursing professionals, which

significantly correlates with an increased level of experienced burnout (Rosales et al.,

2013). The consequences of increased burnout include, but are not limited to, poorer

quality of care rendered and high turnover and emigration rates. This seminar will

initiate discussion between multiple healthcare disciplines and policy-makers not

exclusive to nursing in an attempt to mitigate the negative impact of compassion

fatigue and burnout in Philippine healthcare.

Student nurses are at a favorable position to discuss this topic due to their

capacity to gather speakers educated in this topic to facilitate the discussion and invite

healthcare professionals from the several affiliated healthcare institutions of the Xavier

University College of Nursing. Student nurses and newly-graduated registered nurses,

doctors, and other professions benefit greatly from this seminar by implementing the

methods of reducing burnout and compassion fatigue during the start of their careers

as healthcare professionals.

II. TITLE

This seminar is given the preliminary title of “Burnout and Fatigue in Healthcare:

Restoring the ‘Passion’ in ‘Compassion’”. This title conveys the seminar’s intent to

restore the enthusiasm of healthcare professionals in all aspects of healthcare by

focusing on quality-of-life improvements in the workplace, techniques in effectively

managing occupational stress, and finding significance and meaning in the individual’s

chosen profession in order to improve job satisfaction. Since job satisfaction is a

positive contributing factor to quality of care rendered to patient, it stands to reason

that improving job satisfaction in healthcare will also improve the overall quality of

Philippine healthcare.

III. STATUS AND LOCATION OF THE SEMINAR

The said seminar will be held on March 6, 2017, 8:00AM – 12:00PM at the

Nursing Amphitheatre of Xavier University – Ateneo de Cagayan. The aforementioned

venue has the capacity for 150 participants and with its structure designed for lectures

and large group discussions, making it conducive for the implementation of the

seminar. The group is expecting around 120 invited healthcare workers such as

registered nurses, physicians, barangay health workers, student nurses and medicine

students.

Furthermore, the venue can be adorned with various decorations so that it may

suit the syndicated seminar’s theme and motifs, simultaneously attracting the attention

of the participants.

IV. BENEFICIARIES

The proposed seminar entitled, “Burnout and Fatigue in Healthcare: Restoring

the ‘Passion’ in ‘Compassion’” will be beneficial to the following:

a. Xavier University – College of Nursing Students and School of

Medicine will be able to understand the differences and similarities of

compassion fatigue and burnout as well as the current issues and concerns

regarding the said topics and awareness of how it could affect their future

as health care professionals.

b. The College of Nursing Faculty and Staff will be able to gain insights,

information, and updates with regards to compassion fatigue and burnout

and how they can further discuss these in future classroom settings and

how they can play a role in providing guidance as an experienced

professional.

c. Physicians, Nurses, Barangay Health Workers, and Other Members of

the Health Care Team will be aware of the issues and concerns regarding

compassion fatigue and burnout and how it can affect their roles as part of

the healthcare team. This would also provide them the platform to discuss

what they can do to prevent this from happening in their work environment.

V. BACKGROUND OF SEMINAR

According to Compassion Fatigue Awareness Project (2017), Dr. Charles

Figley emphasized that compassion fatigue is the emotional residue or strain of

exposure to helping people in distress to the degree that it can create a secondary

traumatic stress for the helper. Burnout on the other hand is of different aspect. It is

not trauma-related. Conversely, it is a cumulative process evidenced by emotional

exhaustion and withdrawal associated with increased workload and institutional stress

(The American institute of Stress, 2012). In line with this, the group decided to make

this as the topic of interest in the proposed syndicated seminar.

In accordance with the CHED Memorandum Order No. 14 series of 2009 and

in partial fulfillment of the requirements in Nursing Care Management (NCM) 107.2

Leadership and Management, the level 4 nursing students of Xavier University –

Ateneo de Cagayan College of Nursing, will be conducting this syndicated seminar.

The seminar will run for approximately 4 hours at the Nursing Amphitheatre of Xavier

University – Ateneo de Cagayan. This seminar will provide opportunities for the

students to apply the learned concepts of leadership and management in NCM 107.2

of the first semester academic year 2017 – 2018 and in NCM 108.1, alongside NCM

107.2 of the second semester of the aforementioned academic year. This seminar will

help health care professionals, nursing students, and clinical instructors to be

knowledgeable about compassion fatigue and burnout and on how to overcome these.

This syndicated seminar is grounded from the Sustainable Development Goals

(SDGs) proposed by the United Nations Development Program which is in turn

integrated by different countries into their policies and national development plans

(United Nations, 2015). The said seminar is in line with SDG 3: Good Health and Well

– Being which include substantially increasing health financing and the recruitment,

developing, training and retention of the health workforce in developing countries,

especially in least developed countries and small island developing sates, and the

prevention and treatment and promote mental health and well – being by 2030.

Another is SDG 8: Decent Work and Economic Growth that includes protection of

labour rights and promotion of safe and secure working environments for all workers

by 2030. It is also aligned with the Xavier University Developmental Goals (UDG) on

health, which aims to improve public health through preventive measures of which this

syndicated seminar is all about.

VI. JUSTIFICATION

The term compassion fatigue was coined to describe the phenomenon of stress

resulting from exposure to a traumatized individual rather than from exposure to the

trauma itself. Compassion fatigue is also caused by empathy, which is the natural

consequence of stress resulting from caring for and helping traumatized or suffering

people. The most commonly used definition of compassion fatigue was developed by

Charles R. Figley, who describes the concept as “a state of exhaustion and

dysfunction biologically, psychologically, and socially as a result of prolonged

exposure to compassion stress and all it invokes.”

Professions regularly exposed to the traumatic experiences of the people they

serve, specifically the members of the health care team, are increasingly susceptible

in developing compassion fatigue due to being exposed daily to patients experiencing

various concerns and crises. According to Cocker and Joss (2016), compassion

fatigue may potentially lead to depression, anxiety, and even posttraumatic stress

disorder (PTSD). These conditions are known to increase sickness absence,

psychological injury claims, and job turnover, which negatively impact productivity.

According to Lynch and Lobo (2012) in their study Compassion Fatigue in

Family Caregivers, “The attributes of compassion fatigue as an established

relationship between the caregiver and the patient or the client all associated with the

caregiving role and the psychological and physical responses it arouses.”

Furthermore, they identified system issues or organizational factors such as physical

and emotionally demanding assignments and extra work days as risk factors for

compassion fatigue.

On other hand, Najjar et al. (2009) identified that empathy is vital to the

development of compassion fatigue, as the caregiver must have the ability to perceive

and understand what their patient or client is experiencing and be able to communicate

this understanding. In that study, it emphasized the psychological response to the

conflicting elements of empathy and suffering provides the foundation for ongoing

stress and subsequent compassion fatigue. Thus, compassion fatigue results from the

change in empathetic ability of the caregiver in reaction to the prolonged and

overwhelming stress of caregiving.

Statistics shows that the prevalence of compassion fatigue ranges from 7.3%

to 40% of workers in intensive care settings and 25% to 70% among inexperienced

mental health professionals and is often seen in health professionals among females,

repeatedly witnessing and care for people after trauma. According to the a local study

of Basmayor (2012), about the state of compassion fatigue in the Philippines as

reflected in its prevalence in caregivers in Metro Manila, it can be concluded that

compassion fatigue is experienced by most nurses and physicians in moderate level,

and the most pronounced effect is emotional stress. This kind of sickness is now

evident especially to healthcare professionals, in which they are receiver, listener and

provider of care to patients who are in need and with different conditions. The seminar

entitled, “Burnout and Fatigue in Healthcare: Restoring the ‘Passion’ in ‘Compassion’”,

is incorporated with the theme of BigHero 6 robot character Baymax. This features the

characteristics of Baymax, the “personal healthcare companion” as a benevolent, self-

sacrificing, critically knowledgeable, medically skillful and the service of a big caring

heart; which can be similarly equated to those members of the healthcare team. Unlike

Baymax who’s a man-made robot, these professionals have the human attribute of

emotions which lead to tendencies of exhaustion from their work. The question of “Are

you satisfied with your care?” gives emphasis that caregivers should not only focus on

evaluating the care given to their patients but also the care they give to themselves

holistically.

VII. OBJECTIVES

A. GENERAL OBJECTIVES

This syndicated seminar aims to provide relevant and timely knowledge, skills,

and attitude towards spreading awareness on the current trends and issues regarding

compassion fatigue among health care workers in hospitals, clinics, and nursing

institutions. The group would also be able to comprehensively and systematically

present information regarding the topic with the help of the invited keynote speakers

highly adept in either fields of clinical psychology, psychiatry, human resource

management, or psychiatric – mental health nursing. In addition, this seminar grounds

itself in facilitating social awareness and institutional initiatives to advocate for a better

working environment and staffing as well as imparting the target participants with effect

ways in personally or professionally dealing with compassion fatigue.

Moreover, this syndicated seminar will provide an avenue for fourth-year

student nurses to apply learned concepts on planning, organizing, directing and

controlling. This activity would also help them to establish a stronger foundation and

experience in collaborating through teamwork as well as personal and professional

development.

B. SPECIFIC OBJECTIVES

At the end of the Syndicated Seminar, the group will be able to:

 Familiarize themselves with the appropriate concepts necessary for health care

professionals to know about compassion fatigue.

 Incorporate concepts of psychiatry and/or clinical psychology and psychiatric -

mental health nursing when stressing significance of the following information

in the maintenance of mental health hygiene.

 Employ different creative and reliable strategies for the syndicated seminar in

order to effect sustainable learnings for application in the participants’ daily

working condition.

Skills

 Execute the concepts of nursing leadership and management especially on the

aspects of planning, organizing, directing, and controlling the functions and

demands of the seminar.

 Hone interpersonal and intrapersonal skills in organizing a syndicated seminar

that aims to raise awareness and to provide timely and relevant information

 Practice the principles of strategic, operational, and fiscal planning, time

management, and structures of an organization so as to integrate the value of

responsibilities and boundaries of each member.

Attitude

 Develop teamwork and cooperation within the group and the working

environment as a critical component to the success of this syndicated seminar.

 Integrate the values of cura personalis, Ad Majorem Dei Gloriam, and magis

from the planning to the actual implementation of the syndicated seminar.

At the end of the Syndicated Seminar, the participants will be able to:

 Learn the basic concepts of compassion fatigue, its psychodynamics and/or

pathophysiology, and the related factors that accompany it.

 Correct misconceptions about concepts of compassion fatigue and its

management, considering its physical, mental, emotional, social and

professional effects.

 Be oriented with effective ways and management to cope up with stress in the

workplace especially those working in clinics, hospitals, and nursing

institutions.

 Identify ways in strengthening human resource capacities in order to facilitate

better work environment and to develop institutional measures to prevent

compassion fatigue among its employees.

Skills

 Apply the basic concepts of compassion fatigue, its management and the

institutional policies transpiring into effective and efficient practice.

 Maximize the use of available community and/or institutional resources in ways

of handling compassion fatigue among health care providers.

 Develop ways to transform learned knowledge, skills, and attitude into a

workable approach in modifying related legislations and social initiative for

better work environment and responsibilities.

about compassion fatigue as well as their policies amongst human resources

management in their respective institutions.

 Be empowered with appropriate responses according to one’s awareness and

the availability and condition of supportive resources.

VIII. DESCRIPTION OF SEMINAR

The seminar aims to educate the different working individuals most importantly

those who work in hospitals and in nursing institutes about the existence of

compassion fatigue. Healthcare professionals are invited to listen to experts both in

the nursing and medical field to impart essential insights regarding the reality of

burnout in relation to stress and demands of work. For the second time, we will be

organizing a symposium that would last for four hours starting eight in the morning on

the sixth of March 2018 at Nursing Amphitheatre of Xavier University – Ateneo de

Cagayan. The planning and brainstorming of ideas started on the first week of the

second semester of this academic year wherein ideas were gathered and selected for

the seminar proper.

We will prepare around 150 seats for our audience who mainly compose of

members of the health care team and student nurses. Several committees were

established to take care of different tasks. A formal program flow was devised

reflecting the activities of the day of the seminar with two emcees assigned. Snacks

will be provided as well as the seminar kits for the participants’ note taking purposes.

The technicalities will be taken care of which also includes the visual presentations.

The decorations are matched with the seminar’s theme to create an ambience for a

pleasant learning experience.

IX. Application of Nursing Roles and Management Function

A.) Planning

a. Mission/Vision

Mission - The “Compassion Fatigue” syndicated seminar brings the issue of

decreasing burnouts and fatigue among members of the health care team which they

primarily experience due to increasing number of patients and ineffective staffing

schedules, with an emphasis of knowing how to manage and to follow the ideal

hospital nurse-patient ratio and to create venues in which professionals in both nursing

and medical fields can interact with students, healthcare professionals, and the

general public in order to reduce the incidence of burnout and fatigue.

Vision - The “Compassion Fatigue” syndicated seminar helps to address the irony of

the situation of the members of the health care team who are giving out care, yet fail

to fully care for themselves, neglecting their overall health. Also, this seminar aims to

help health care providers learn about the management and realities of compassion

fatigue and burnout through the awareness of an informed public and policy-makers.

b. Philosophy

The Xavier University College of Nursing believes in the individual Filipino’s

responsibility to their health and ability to adopt health-promoting and disease-

preventing behaviors through proper health education. The College of Nursing seeks

to develop nurse-leaders that will effect this change, making relevant health

information available to all sectors of society through the use of their innate qualities

and God-given talents.

c. Goals

The following are the goals of the proponents of this seminar

1. Be knowledgeable regarding the management and realities of burnout

among health care providers

2. Give substantial education on the consequences of neglecting the overall

heath among the health care providers

3. Significantly decrease the gaps in our knowledge and understanding of

compassion fatigue and burnout

4. Chance to discuss integrated programs, management and strategies to

reduce incidences of compassion fatigue and burnout

5. Hone effective and efficient management and organization skills of

expected tasks

6. Develop amicable relationships and collaboration among members of the

group

7. Practice the principles of prioritization, competence, leadership, and time

management as the group makes decisions and gives precedence to

necessary responsibilities

d. Policies and Guidelines

The policies applied in order to ensure proper preparation and implementation of the

seminar will be as follows:

1. Attendance will be checked every group meeting. Those marked absent will

be noted on the attendance sheet unless he/she is excused. Invalid

absences will be fined 50 pesos. Those who are late within 15 minutes will

be fined 20 pesos. Failure to appear 15 minutes after the designated

meeting time will be considered an absence.

2. The group treasurer is tasked with collecting the weekly due of the agreed

amount. He/she is responsible for documenting the breakdown of expenses

(liquidation). The collection will start on the 3rd week of November and will

end on the 2nd week of February.

3. Letters of solicitation are strictly prohibited and considered invalid as per the

recommendations of the clinical instructors.

4. Each member will be assigned a specific task by the group leader. Before

final assignment, each member has the right to freely express his/her

concerns which must be addressed appropriately by the heads of each

committee.

5. Each member is equally responsible for any and all revisions made during

the final submission of the written output. If the output is not passed in a

timely manner, the group will meet and decide on the possible

consequences.

6. Though assigned to different committees, all members may participate in

ALL preparatory activities even if not designated by their respective

committees. The committee head may consult another committee head for

permission when soliciting the assistance of a member not belonging to their

own committee.

7. Unity, collaboration, and honesty will be given due emphasis. All members

are encouraged to express any dissenting opinions on any issue.

8. Handling of finances will be subjected to the following additional guidelines:

by item, amount, and price. This is to be approved by the group

leader (Beannca Angeles) and the committee head (Quennie

Uyguangco)

 All official receipts with the name: “XUCN- SS 1 (NA)” will be given

to the Finance Officer for proper liquidation purposes.

 Any excess funds even single centavo must be returned back to

Finance Officer; excess funds after all liquidated costs will be

apportioned and reimbursed to the group members.

e. Function of Each Committee


a. Secretariat

This committee is responsible for communication and


correspondence. They are responsible in contacting and communicating with
the guest speakers and in keeping track of the attendance, ensuring
meetings are organized and timed. With the said task and responsibilities,
they are then responsible for maintaining effective records and safe keeping
of all documents, including the formal letters and responses of any messages
that were sent. Reservation of the venue and materials to be used during the
seminar will also be delegated to this committee.

b. Program Committee

Creation and planning of the program flow during the day of the
seminar is the main task of this committee, including the specific duration of
each activity and crowd control. They will spearhead the practices and
performance of the intermission number(s). Members of this committee are
also the one who will assign other tasks to each member from different
committees, including assigning of emcees, prayer leaders and performers
as well as the preparation of certificates and tokens that will be given to the
audiences and speakers and ushering of the audience and guest speakers.

c. Food committee

This committee is designated to choose and purchase the selected
snacks and refreshments to be distributed. They are also in-charge of
distributing these the audience, clinical instructors, and guest speakers.
Preparation of garbage bags and ensuring maintenance of clean
surroundings are also highly encouraged. Coordination of both food
committee and finance officer is also done to settle the allotted budget for
food.

d. Finance Officer

She will primarily oversee the financing and budgeting aspects of this
seminar. She will spearhead the collection and safekeeping of money
including the receipts of each expense. They will collect funds from the
penalties incurred by each members from failing to follow discussed policies
and guidelines. A formal breakdown and liquidation of the expenses will also
be shown and discussed to the whole group.

e. Physical Set-up Committee

Decoration of the said venue and making it suitable for the theme and
motif is the responsibility of this committee. They are the one who will
conceptualize and facilitate the creation of props and decorations for the
program with the help of all the members of the group. Collaboration with the
program and secretariat committee is also done for planning of the layouts
and design of the media campaigns, posters, invitations, certificates, and
plaques to be distributed.

f. Technical Committee

This committee is focused on ensuring that the flow of the program is


error-free and smooth while preventing the occurrence of a dead air during
the program proper. They are in-charge of the manipulation of the
Powerpoint presentations, sound effects, visuals, and lighting. They should
also coordinate with the laboratory technician for further troubleshooting and
dry-runs.

c. Speakers

The students of Level 4 block NA have decided possible speaker in the

seminar. Possible first speaker, Dr. Maria Nena Peñaranda, MD, DPBP, is a doctor of

Psychiatry and Behavioral Medicine, Member of Philippine Psychiatry Association.

She had conducted previous talks regarding topic on burnout among Healthcare

providers and currently a Faculty of Dr. Jose P. Rizal School of Medicine. As a first

speaker, they will provide relevant details and discuss mainly on the topic on burn-out;

what is burn-out, why is it happening, what are the management or ways to cope up

with it. The second speaker will be either, Sir Hashley Kynt C. Maape, RN, OIC Chief

Nurse at Misamis Oriental Provincial Hospital – Magsaysay , Previous Head Nurse at

MRXUH by which He conducted a research study among his unit regarding the

prevalence of Compassion Fatigue, and had also Conducted Previous talks regarding

topic on Compassion Fatigue among Healthcare providers who will be discussing on

the behalf of health care providers about how compassion fatigue affect a certain

health professionals. He will also discuss its prevalence, managements and strategies

to cope up from this compassion fatigue. The above-mentioned speakers were chosen

because of their knowledge on dealing a person with compassion fatigue that will

enable the participants of the seminar to be informed and knowledgeable about the

topic.
