The Ashanti Region of Ghana has beenidentified as having the second highest prevalence rate of

3.9% among HIV positive pregnant women attending antenatal care. This raises concerns due to

the possibility of transferring the virus to their babies during pregnancy, labour and delivery and

through breastfeeding. Though it is a requirement for every pregnant woman to be screen for

HIV, none availability of men accompanying their pregnant partners to antenatal clinics raises

another issue regarding disclosure HIV status by pregnant women. The study was therefore

conducted to assess factors associated with HIV/AIDS spousal disclosure among pregnant

mothers in the Kumasi Metropolis. The study adopted a cross-sectional design employing a

mixed methods approach to data collection. Questionnaire were used in eliciting data from a

sampled 260 HIV positive pregnant women attending PMTCT or ART services at the Suntreso

Government Hospital and the Kumasi South Hospital at the time of the study. STATA SE version

12 for used to for data processing. A little over half (51.2%) of the participants have disclosed

with majority (48.8%) not knowing how long it took them to disclose to their partners. Aside

partners, 45% have disclosed to their relatives. The HIV positive status of respondents have not

significantly changed the sexual behavior of respondents as 15% still keep other sexual partners;

68.8% never or sometimes use condoms during sexual intercourse and 63% will never refuse

their partners sex without condoms. 66.4% of respondents do not know the HIV status of their

partners. More than half (52.3%) indicated knowledge of their partner’s HIV have not influence

on their HIV status disclosure. Fear of partner’s withdrawal; withdrawal of financial support;

worry about other people finding out about HIV status and fear of partner’s physical violence

were cited as factors serving as barriers to disclosure. It is recommended that the Ministry of

Health through its agencies as well as NGOs on health, develop social marketing campaigns that
encourages HIV positive pregnant women on the benefits of disclosure as well as encouraging

them on the need to condom use during sex.
