Ako, si JUAN DELA CRUZ, nga nasa saktong edad, minyo, Pilipino, na
nagpuyo sa Marfa, Bakilid II, Mandaue City, Philippines, pagkahuman ug
panumpa sumala sa balaod, mosaysay niining mosunod:

(I, JUAN DELA CRUZ, of legal age, married, Filipino citizen, and a resident of
Osmeña Street, Cebu City, Philippines, after having been duly sworn in accordance
with law, do hereby depose and say that:)

1. Ako, usa ka Truck Driver sa Lakambini Enterprises Inc.(Lakambini)

sukad Oktubre 2, 1998;

(I am a Truck Driver of Lakambini Enterprises Inc. (Lakambini) since

October 2, 1998;)

2. Isip usa ka Truck Driver, ako, uban sa duha ka mga kauban isip mga
kargador, maoy mo deliver sa mga produkto sa Lakambini;

(As a Truck Driver, I, together with two others who work as Truck
Helpers, are the ones who deliver the products of Lakambini;)

3. Usa sa kanunay nakong mauban, mao si DIOMEDES MAMIDPID

(DIOMEDES) nga nagtrabaho usab sa FPGMC sukad niadtong Enero
15, 1996;

(One Truck Helper who is usually with me at work is DIOMEDES

Mamidpid, who also worked in FPGMC since January 15, 1996;)

4. Sa tanto namong panag-uban sa trabaho, nakahibalo ko nga sa kadugayon

sa panahon, si DIOMEDES naay gipamati sa iyang mata;

(Since we have been together at work, I knew that for quite some time,
DIOMEDES was having problem with his eyes;)

5. Ni adto lamang Septembre 15, 2016, ni ingon siya nako nga nananghid
siya sa FPGMC nga mo absent una kay siya magpa opera sa iyang mata;
 (It was only in September 15, 2016, he told me that he asked PGMC that
he will be absent from work because he will be having an eye surgery;)

6. Ang maong petsa (Septembre 15, 2016) wala nako hikalimti kay ang
sunod nga adlaw niana maoy pagkamatay sa akong anak;

(I did not forget the said date (September 15, 2016), because the next day,
was the day my child died;)

7. Akong nahibaw.an nga si DIOMEDES ka duha na operahan sa iyang

mata;

(I was informed that DIOMEDES did two eye surgeries;)

8. Nahibalik lamang si DIOMEDES niadtong Hulyo tuig 2017 human

magpa-ayo sa iyang samad kay mao man ang mando sa doktor kaniya;

(DIOMEDES only came back sometime in July of this year (2017) after
his recovery because that was the advice of the doctor;)

9. Akong ipahayag dinhi, ubos sa silot sa salang perjury o pagpamakak kon

mapamatud-an nga ako namakak, nga kining tanang nahisulat dinhi
akong gibuhat ug gi-usisa sa tinudanay gayod. Kining tanang nahisulat
dinhi matuod ug husto segun sa akong nahibaloan ug pagtuo.

herein are made and verified by me in good faith. These entries are true
and correct to the best of my knowledge and belief.)
