Pursuant to the Public Information Act, Texas Government Code, Chapter 552, please provide to me copies of the TCOLE
transcript — often referred to as a “Personal Status Report” or “PSR” — that includes award history for licenses and
certificates; service history; licenses; formal education; and continuing education and training history for the following
persons:

1. Samuel Lee Allen, born and a resident of Cedar Hill, Dallas County, Texas, with likely Texas drivers license
(“TXDL”) number

2. Kevin Lugene Solomon, born and a resident of Irving, Dallas County, Texas, with likely TXDL number

3. Bryan David Woodard, born and a resident of DeSoto, Dallas County, Texas, with likely TXDL number

4. Edward Raynard Wright, born and a resident of DeSoto, Dallas County, Texas, with likely TXDL number

5. Donald Grigsby Artis, born and a resident of DeSoto, Dallas County, Texas, with likely TXDL number

6. Hershalyn “Sha” J. Steger Knight, born and a resident of DeSoto, Dallas County, TX, with likely TXDL
number

If one or more of the individuals do not currently and have never held licensing with TCOLE and thus a PSR does not
exist, please indicate as such in your response.

I stipulate that any information that is confidential by law (e.g. Social Security number, residential address, personal cell
phone number, etc.) can be redacted from the provided copies without need for an opinion, ruling, and/or decision from
the Office of the Texas Attorney General.

If some of this material is more readily available than the rest, either because of logistics or questions of law, I request
the release of the available material while the remaining items are being prepared or reviewed for release.

In order to reduce the cost for and expedite the production of these records the aforementioned documents are
requested to be supplied via email attachment in an electronic copy in a standardized readable format such as Adobe
PDF.
