Has the Pilot card been completed and are the Pilot embarkation arrangement in hand ?

Have VHF channel for the various services (e.g. VTS, Pilot, tugs, berthing instruction)
been noted and a radio check carried out ?

Has the port been fully aware of any special berthing requirements that the ship may have ?

have flags/light signals been exhibited prior to arrival ?

has notice been given to deck crew for station ?

GMDSS CHECKLIST

> Correct identity numbers in all the equipment in
accordance with thw radio licence, namely: VHF DSC

EPIRB and INMARSAT SES.

2. Radio log and Verify that radio log and relevant records are permanently
records updated and maintained for :
> Communications relating to distress, urgency and safety
of traffic.
> Incidents connected to radio services of importance to

> Details required by radio equipment e.g. Daily weekly,
monthly test etc.
> Details of battery maintenance (see item B).

3. Radio Publications Verify that up-to-date ITU publications are onboard.

if at sea maintenace ie. The declared option.

8. Source of energy Examine main source of power supply to the radio


installation and charger of the batteries used as reserve
source of energy.
Examine the reserve source of power supply. If battery
operated, check for good condition of batteries by specific
gravity or voltage measurement, adequate capacity to
operate basic or duplicated equipment for the required
duration, battery voltage and discharge current with the
battery of-charge and on load, and check the means for
automatic charging.

1605 KHz and 4000 KHz, or 4000 KHz and 27500 KHz).
If Fitted, check the operation of MF/HF instalation on :
> Distress & safety frequencies using DSC, radiotelephony
and direct printing between 1605 KHz and 4000 KHZ and
between 4000 KHz and 27500 KHz.

11. INMARSAT ship Check the operation of INMARSAT ship earth station in
earth station particular :
> Verify the distress function by means of an approved
test procedure, where possible.

12. Other GMDSS Check the operation of EGC facilities through incoming
message or running the self-test program if provided.
Check the operation of HF NBDP receiver through
incoming message or running self-test program if provided.

13. NAVTEX receiver Test the NAVTEX receiver by incoming message or


running the self-test program if provided.

14. Satellite EPIRB Inspect EPIRB for defects, and proper position and
mounting for float free operation.
Check condition and expairy date of the battery and
hydrostatic release device.
Self-test routine to be carried out according to the
manufacturers instructions.
Verify that ship's identification number is clearly marked
outside th EPRIB.

15. Two-way VHF Check two-way VHF radio apparatus for :


apparatus > Proper operation on both Channel 16 and any other
channel through a test with another fixed or portable VHF
instalation.
> Battery-charging arrangements and/or expiry date of
date of battery.

16. Search and Rescue Check radar transponder(s) for :


Radar Transponder > Proper position and mounting.
> Response on ship's 9GHZ radar and battery expire date.

17. Operational Verify that all radio watch-keeping personnel understand
procedures and can demonstrate how to initiate a distress alert on all
equipment and conduct self-test of all equipment in
particular :
> Verification of the operation of MF or HF/MF DSC
controller by means of test call on HF and/or HF to
a coast station, if provided.

18. Position Updating Verify that postion updates to VHF DSCs, MF DSC and/
Receiver or MF/HF DSC, INMARSAT-C SES have ben provided
from external position receiver or INMARSAT-C SES with
in-built GPS receiver.
Check presence and condition of backup battery if
external position receiver is being used to ensure supply
of uninterrupted position information to all the equipment
capable of providing 2-way communications as mentioned
above.

19. Maintenance Confirm that adequate methods to ensure maintenance


Requirements are available and applied such as :
> Duplicated equipment, validity of teh contract with a
maintenace company or adequate manning.

20. Qualification of Check if radio operators hold appropriate GMDSS

COMPANY'S BRIDGE STANDING ORDER PERIN

GENERAL UMUM

1. The Company's bridge standing orders are to be read by all deck officer on 1.
first joining the ship and before taking over at sea watch. These rules shall be
observed whilst at sea and at anchorage and the officers shall acknowledge
having read and understood them by signing on the front page of this booklet.
2. The Officer Of the Watch (OOW) is the Master's representative and is
accountable to him for his conduct while on watch. His primary responsibility
at all times is the safe navigation of the ship, safety of life and protection of 2.
the marine environment. The OOW shall at all times comply with the
"International Regulations for Prevention Collision at Sea" and all guidelines
as laid down in the "Bridge Procedures Guide".
3. It is important that the OOW maintains an efficient lookout at all times. Visits
to the communication centre (GMDSS area) and chart table shall be brief,
particularly during the night, in heavy traffic area, when small fishing boat are
in the vicinity and during inclement weather. 3.
4. The OOW shall bear in mind that the main engines are at his disposal for
assistance in manoeuvring. He should not hesitate to use them in case of
need, although timely notice of an alteration of engine movements should be
given when possible. The OOW shall also be fully aware of the manoeuvring
capabilities of his ship including rate of turn at various helm angles and 4.
stopping distance in case of engine breakdown.
5. The OOW shall bear in mind that the ship's sound signaling equipment
(whistle) is available for use as required under the regulations for preventing
collision at sea.
6. The OOW shall check for incoming messages at hourly intervals during his
watch and advise the Master accordingly.

CHANGE OVER OF THE WATCH

1. The officer of the watch should not hand over if he has any reason to believe 6.
that the relieve officer is suffering from disability (including illness, alcohol,
drug, or fatigue), which would prevent him from carrying out his duties
effectively. If in doubt, he should consult the Master.

2. The relieving officer shall not take over the watch until his vision is fully PERGANT
adjusted to the existing light conditions and has personally satified himself of
the following : 1.
i) Read and understood night orders and other additional master's
instructions related to that voyage.
ii) Verified ship's position, course, speed, under kell clearance and
visibility and traffic in the vicinity.
iii) Operational condition of the main navigational equipment (gyro
compass, magnetic compass, autopilot, radar, GPS and echo 2.
sounder).
iv) Navigational marks and other navigational situations including,
but not limited to the operational condition of the steering gear
and safety equipment being used or likely to be used during the
watch, error of gyro and magnetic compasses.
3. The relieving officer of the watch shall ensure that members of his watch are
fully capable of performing their duties particularly with regards to their
adjusment to night vision.
4. If during the watch, the officer being relieved is involved in a manoeuvre or
other action to avoid any dangers, then the relieving shall be delayed until
manoeuvre is complete, ship is out of danger and back on a steady course.
5. After handling over the watch, the relieved office shall carry out rounds within
the accommodation to check for fire, flooding and other unusual
circumstances and report his finding to the OOW. 3.

CALLING THE MASTER

The officer of the watch shall immediately notify the Master in the following
circumstance :
i) If visibility detoriorates below the level laid down in the Master's Standing
orders. 5.
ii) If the traffic condition or movements of other ships are causing concern.
iii) If difficulty is experienced in maintain course.
iv) On failure to sight land, a navigational mark or to obtain soundings by the
expected time.
v) If unexpectedly land or a navigational mark is sighted or unexpected MEMANG
sounding is observed.
vi) Breakdown of engine, teering gear and any essential navigational equipment. Mualim Ja

takes over, the officer of the watch is responsible and shall take such immediate vii)

PERIODIC CHECK OF NAVIGATIONAL EQUIPMENT ix)

1. Operational tests of shipboard navigational equipment shall be carried out x)


at sea on a regular basis and their accuracy checked and recorded in the
logbook or the record book as and when circumstances permit. xi)
2. During the watch, the OOW shall ensure that :
i) The autopilot or the helmsman is steering the correct course. xii)

HELMSMAN/AUTOPILOT STEERING

Attention of the OOW is drawn to the requirements of Regulation 19 Chapter V 2.


of Solas 1974 on above subject. In conditions of high traffic density, narrow
channels and restricted visibility, the OOW shall have available a qualified
helmsman, ready at hand. So that change-over from autopilot to handsteering can
be carried out in good time, without affecting contuinity of the lookout and allow
for any potentially hazardous situation to be dealt with in safety. The change-over
from autopilot to hand steering should be made under the supervision of
responsible officer.

NAVIGATION IN COASTAL WATER

1. When navigating in coastal areas, the largest scale chart, which has been
corrected up to date with the latest information available, shall be used.
2. All objects used for position fixing shall first be positively identified. GPS
should be used to asist in this.
3. Position fixing shall generally be done at 15-minute intervals and at alteration
points. The Master may direct position fixing to be done at shorter intervals
than above at his discretion.

1. NAVIGATION WITH PILOT ON BOARD

Presence of the pilot on the bridge does not relive the Master of his responsibility
for the safe navigation of the ship. The OOW shall co-operate closely with the pilot
and maintain frequent checks on the vessel's position and movements. If the OOW
is in any doubt as to pilot's actions or intentions, he shall inform/consult the Master
and (if Master is not on the bridge at the time) in his absence seek clarification from
the pilot. If still in doubt, he shall notify the Master immediately and take such
action(s) as is necessary before the Master arrives (IMO resolution A.285-VIII).

2. SHIP AT ANCHOR

A Continuous anchor watch shall be maintained at anchor. In all circumstances


whilst at anchor, the officer of the watch shall :
i) Check the ship's position on the appropriate chart at regular intervals
particularly during and after change of tide, the range and bearing of targets
used to be logged in the anchor bearing book.
ii) If anchor drag is observed, notify the Master and take immediate remedial
action(s) including calling forward stations and informing engine room to
prepare main engines for movements.
iii) Ensure that an efficient visual lookout and radio listening watch is maintained
at all times.
iv) Ensure that anti-piracy and anti-theft precautions are taken as laid down in
Master's standing and night orders.
v) Regular inspection rounds in the accommodation are made for fire safety and
that unauthorised crafts are not allowed to stay alongside or in the vicinity.
vi) Observe vicibility, other meteorological conditions (sudden squalls/sumatras)
and notify Master if they affect ship's safety.
vii) Ensure that appropriate lights and shapes are displayed and appropriate
sound signals sounded/made.
In case of an emergency, the duty officer shall act in accordance with ship's
contingency plan as identified in SMSO Vol. 1.

PERINTAH TETAP UNTUK JAGA DI ANJUNGAN DARI PERUSAHAAN

UMUM

Perintah Tetap untuk jaga dianjungan dari Perusahaan harus dibaca oleh
semua Mualim yang baru bergabung dikapal dan sebelum melakukan
serah terima tugas jaga dilaut. Aturan-aturan ini harus dipatuhi selama
kapal berlayar dilaut dan berlabuh jangkar dan para mualim menyatakan
bahwa telah membaca dan memahami untuk kemudian menanda
tanganinya dihalaman depan dari buklet ini.
Mualim Jaga adalah wakil Nakhoda dan bertanggung jawab kepadanya
selama melakukan tugas jaga. Tanggung jawab utamanya setiap waktu
adalah melakukan navigasi yang aman, keselamat jiwa dilaut dan per
lindungan terhadap lingkungan maritim. Mualim Jaga setiap waktu harus
mematuhi "Regulasi Internasional untuk Pencegahan terhadap Tubrukan
di Laut" dan semua pedoman sebagimana tercantum dalam "Pedoman
Prosedur Jaga Dianjungan".
Adalah penting bahwa Mualim Jaga harus menempatkan pengamat yg
efektif setiap waktu. Mendatangi lokasi komunikasi (area GMDSS), meja
peta harus dilakukan secara singkat khususnya pada waktu malam hari,
diperairan yang kepadatan lalulintasnya tinggi, apabila melihat banyak
perahu penangkap ikan disekitar kapal dan selama cuaca buruk.
Mualim Jaga harus selalu mengingat bahwa mesin induk adalah bagian
yang membantunya untuk melakukan olah gerak. Jadi ia tidak perlu ragu
utk menggunakannya jika diperlukan, walaupun pemberitahuan secara
tepat waktu kekamar mesin terhadap perobahan putaran mesin harus
dilakukan jika waktunya memungkinkan. Mualim Jaga harus mengetahui
kemampuan olah gerak kapalnya termasuk berbagai gerakan sudut
kemudi dan jarak henti dalam hal mesin mengalami kerusakan.
Harus selalu ada dalam ingatan Mualim jaga bahwa peralatan sinyal
suara (suling kapal) siap untuk digunakan sesuai dengan regulasi untuk
pencegahan tubrukan dilaut.
Mualim jaga harus selalu memeriksa berita yang masuk dengan interval
setiap jam selama jagaannya dan berikan saran yang sesuai kepada
Nakhoda.

PERGANTIAN JAGA

Mualim jaga tidak akan menyerah tugas jaganya kepada penggantinya,


bila diketahui bahwa Mualim yang akan menggantikannya tidak mampu
melakukan tugas jaga (termasuk sakit, alkohol, narkoba atau menderita
kelelahan), dengan mencegah Mualim jaga pengganti untuk melaksana
kan tugasnya secara efektip. Jika terjadi keragu-raguan, harus segera
dikonsultasikan dengan Nakhoda.
Mualim jaga pengganti harus tidak menerima penyerahan tugas jaga
sebelum penglihatannya menyesuaikan dengan kondisi cahaya yg ada
dan secara personal yakin bahwa akan mengambil alih tugas setelah :
i) Membaca dan memahami perintah malam dan instruksi tambahan
lain dari Nakhoda yang ada kaitannya dengan pelayaran yg sedang
dilakukan.
ii) Periksa posisi kapal, haluan, kecepatan, jarak antara lunas kapal
dengan dasar laut dan lalu lintas disekitar kapal.
iii) Kondisi operasi dari peralatan utama navigasi (gyro compass,
magnetic compass, autopilot, radar, GPS dan echo sounder).
iv) Benda-benda navigasi termasuk situasi navigasi yang lain, tapi tidak
hanya terbatas kepada kondisi navigasi pengunaan peralatan
kemudi dan penggunaan peralatan keselamatan saja atau yang
mungkin digunakan selama jaga, error dari gyro dan kompas magnit.
Mualim jaga pengganti harus memastikan bahwa regu jaganya sepenuh
nya mampu khususnya yang berkaitan dengan penyesuaian penglihatan
mereka pada malam hari.
Jika saat penjagaan, Mualim yang akan diganti sedang melakukan olah
gerak atau tindakan lain untuk menghindari bahaya, serah terima tugas
ditunda sampai dengan tindakan olah gerak telah selesai, kapal sudah
sudah bebas dari bahaya dan kembali kehaluan semula.
Setelah menyerah terimakan penjagaan, Mualim yang diganti harus
melakukan ronda keliling disekitar akomodasi kapal untuk memeriksa
kemungkinan adanya kebakaran, banjir dan segala sesuatu yang tidak
biasa lainnya dan laporkan penemuannya kepada Mualim Jaga.

MEMANGGIL NAKHODA

Mualim Jaga harus segera memberitahu Nakhoda jika keadaan sebagai


berikut :
jika jarak tampat memburuk sampai dibawah tingkat yang ditetapkan
dalam Perintah Tetap Nakhoda.

Jika kondisi lalulintas atau pergerakan kapal lain yang mengkhawatirkan


Jika mengalami kesulitan untuk mempertahankan haluan.
Terjadi kegagalan untuk melihat daratan, tanda navigasi atau pengukur
an kedalam laut pada waktu yang telah ditetapkan.
Dengan tak terduga terlihat adanya daratan atau tanda navigasi atau
dengan tak terduga pula diamati adanya suatu kedalaman.
Adanya kerusakan mesin, mesin kemudi dan setiap peralatan navigasi
yang penting
Jika peralatan radio tidak berfungsi.
cuaca meningkat menjadi buruk atau setiap keadaan cuaca yang me
mungkinkan dapat menjadikan kerusakan dikapal.
Diterimanya berita bahaya yang datangnya dari kapal lain yang ada di
sekitarnya atau terlihat kapal yang sedang mengalami bahaya.
Kietika perilaku kapal lain menimbulkan kecurigaan khusus didaerah
yang sering terjadi pembajakan.
Jika kapal bertemu dengan setiap bahaya navigasi seperti gunung es
atau kapal yang ditinggalkan.
Setiap waktu adanya keragu-raguan.
Apabila Nakhoda sudah diberitahu, dibutuhkan waktu untuk tiba dianjungan
untuk mengambil alih komando, dan dalam waktu tersebut Mualim Jaga
tetap bertanggung jawab dan harus mengambil tindakan segera yg mungkin
diperlukan untuk menghindari atau mengurangi dampak dari bahaya.

PEMERIKSAAN SECARA BERKALA TERHADAP PERALATAN NAVIGASI

Pengujian secara operasional terhadap peralatan navigasi dilakukan


secara teratur dilaut dan keakuratan pemeriksaan dan dicatat dalam
buku harian atau dalam buku catatan jika waktu memungkinkan.
Selama waktu jaga, Mualim Jaga harus memastikan bahwa :
i) Kemudi otomatis atau jurumudi yang mengemudikan dengan haluan
yang benar.
ii) Secara berkala dibandingkan antara pedoman standard dengan
pedoman gyro.
iii) Apabila menggunakan kemudi otomatis, kemudi harus dicba ke
mode manual satu kali setiap penjagaan.
iv) Salah tunjuk dari pedoman standard dan pedoman gyro harus diten
tukan tidak kurang dari satu kali setiap penjagaan dan setelah me
lakukan perobahan haluan dengan sudut yang besar.
v) Semua lampu navigasi dan lampu sinyal dinyalakan dalam kurun
waktu dari matahari tenggelam sampai dengan matahari terbit dan
pada saat jarak tampak terbatas.

by taking bearing of fixed navigation marks or readily identifiable shore objects

ensure that proper look-out is maintained & that rounds of the ship are regularly made

observe metrological and tidal conditions and the state of sea


notify the Master and under take all necessary measures if ship drags anchor

ensure that the state of readiness of the main engines and other machinery is in
accordance with the Master's instruction
If visibility deteriorates, notify the Master

ensure that the ship exhibits the light/shapes and sound signals, as appropriate

take measures to protect the invironment from pollution by the ship

UNDER KEEL CLEARANCE POLICY

The Company's Under Water Keel Clearance Policy to provide safe navigation for
the Own ship's through the areas restricted available depth of water and thus
enhancing safety of ship and protection of environment.

Under Keel Clearance is estimated from some or all the following factors :

> The vessel manoeuvering characteristics.


> Accuracy in the assessed vessel's draft including influence of water density and
hogging or sagging.
> The vessels squat and or settlement (related to vessel's speed).
> Increase in draft due to vessels trim.
> Increase in draft due to vessels roll.
> Increase in draft due to vessels pitch.
> Increase in draft due to vessels heave.
> Accuracy in the predicted or measured tide height or Mean Sea Level.
> Accuracy in the declared charted depth.
> Allowance for possible siltation since last survey.

RESPONSIBLE
The Master is responsible for estimating the minimum under clearance along the
whole transit route of the vessel, including the port facility or anchorage.
Prior berthing to port facility with port Pilot. The ship Master and the relevant Pilot
shall discuss and agree the plan to anticipated under keel clearance.

MINIMUM UNDER KEEL CLEARANCE

PT. Cakra Bahana require that all vessels to maintain minimum under keel
clearance as follows :

> When operating in open sea at least a minimum 3.0 meters Under Keel
Clearance (UKC) to be maintained.

> When approaching shallow waters and at controlled, practice at least


1.0 meters minimum Under Keel Clearance.

> When at terminal / during loading or discharging, vessel to maintain


minimum UKC of 0.5 meter at all times

Where a Government, Port Authority, or Pilot organization establishes a mandatory


or recommendation of minimum Under Keel Clearance, the Master must ensure
that, as minimum, such Under Keel Clearance is maintain after taking in to account
the factors listed above.
