“RESOLUTION TO OPEN A CHECKING ACCOUNT WITH AN ACCOUNT NAME OF

DSWD KC NCDDP BARANGAY POBLACION IN THE LANDBANK OF THE
PHILIPPINES TANDAG BRANCH AND AUTHORIZING THE FOLLOWING AS
SIGNATORIES: BSPMC CHAIRMAN LINDA D. TOMALON, BARANGAY TREASURER
LORENA P. CANCIO, AND KALAHI CIDSS NCDDP AREA COORDINATOR ERAHJANE
CANDA”

WHEREAS, Kalahi CIDSS NCDDP is a special project funded by the Department of

Social Welfare and Development (DSWD) CARAGA Region has already identified their
project which is Construction of 442 ln.m. Drainage Canal for the Community
Volunteers of POBLACION;

WHEREAS, the project has undertaken its implementation activities and given the
authority to the Barangay Sub Project Management Committee (BSPMC) Chairman
LINDA D. TOMALON, Barangay Treasurer LORENA P. CANCIO and Kalahi CIDSS NCDDP
KC NCDDP AREA COORDINATOR ERAHJANE CANDA as official signatories;
NOW THEREFORE BE IT RESOLVED, as it is hereby resolved in the session to open the
current account in the Land Bank of the Philippines – Tandag Branch with the account
name KALAHI CIDSS NCDDP/BARANGAY POBLACION;

RESOLVED FURTHER, that the authorized Barangay Sub – Project Management

Committee (BSPMC ) Chairman LINDA D. TOMALON, Barangay Treasurer LORENA P.
CANCIO and Kalahi CIDSS KC NCDDP AREA COORDINATOR ERAHJANE CANDA as
official signatories can make use of the account and represent themselves as
signatories for any financial and bank transactions;

RESOLVED FURTHER, to furnish copies of this resolution to the Land Bank of the
Philippines - Tandag City SDS Branch, the DSWD RO XIII, BSPMC Office and other
concerned agencies for their information and appropriate action;

APPROVED, adopted this 8th day of JANUARY, 2018 at BARANGAY POBLACION,

Marihatag, Surigao del Sur upon motion of Hon. Ricardo Pontillo and duly seconded by
all present.
