ANNEX B

I. Requirements for Permit to Operate Air Pollution Installation (New Application)

1. Duly Accomplished Application Form

2. Engineering Report prepared, signed and sealed by a Mechanical Engineer and duly notarized containing the
following:

 Plant description and operations

 Estimated types, concentrations and quantities of emissions

 Proposed control facilities, emission rate and annual mass emission

 Design criteria of air pollution control equipment (APCE) used and its efficiencies including the quantities
at the final emission

3. Plans and specification of installations and its control facilities in standard size of 50cm x 90cm/Picture of the
Installation

4. Air quality impact analysis showing the resulting ambient air concentration for significant pollutants from the
facility, including the existing ambient air concentration as baseline

5. DTI Registration
6. Vicinity map

7. Copy of Environmental Compliance Certificate (ECC)

8. Designation of Pollution Control Officer (PCO)

9. Letter of appointment/designation as PCO of the establishment;

10. Accomplished PCO Accreditation Application Form;

11. Curriculum vitae with ID Picture of the appointed/designated PCO;

12. Notarized Affidavit of Joint Undertaking of the PCO & the Managing Head;

13. Photo copy of Official Transcript of Records / Diploma;

14. Certificate of Training for forty (40) hours of Basic PCO Training per recent issued DAO 2014-02;

15. Certificate of Training for Eight (8) hours of the Managing Head/Owner on Environmental Management
covering the basic environmental laws.

Continuation of Annex B

1. the name of the receiving body of water and its official water classification and in case of land discharge, the
nearest receiving body of water and its official water classification;

2. information on flow measurement and equipment and procedure;

3. Pollution Prevention and Environmental Management System plan or program;

4. DENR ID Number as hazardous waste generator (if applicable);

5. statement of the cost incurred in the installation and maintenance of wastewater treatment facility, if any;

6. quality and quantity of abstracted water; and

7. copy of the Environmental Compliance Certificate (ECC) from the Department or Certificate of Non-Coverage
(CNC) as applicable.
