DETAILS OF THE INVESTIGATION:


On about 1221H 03 September 2015, SFO3 Librado Villanueva, Duty Operator of Makati
City Fire Station, received an emergency call from a certain Pedring Datur informing that fire is
in progress at #8236 San Jose St., Brgy Guadalupe Nuevo, Makati City. Upon receipt of said
call, duty operator immediately dispatched Engine 02 followed by other firefighting units of this
City at around 1222H.

Upon arrival at the fire scene, thick black smoke was seen from the third floor of said
residential house prompting personnel of Engine 02 headed by SINSP ROWENA H GOLLOD
BFP, Guadalupe Nuevo Fire Substation Commander to tap First (1 st) alarm at around 1230H
and started the firefighting operation. Second (2 nd) alarm was raised by SINSP GOLLOD at
around 1241HH. CINSP RODOLFO P DENAGA, Chief, Operation Section of this city arrived
and manned the firefighting operation. The latter assessed that said fire was prevented to
spread at the nearby structures prompted him to announce “Fire-Under-Control” at about
1255H.

The Officer-In-Charge, CINSP GREGORIO S ELBAMBO BFP, responded at the fire


scene and directly supervised all the responding units and fire operations. Finally, he declared
fire out at exactly 1305H.

The involved structure is a three (3) storey residential house made up of mix concrete
walls, wooden materials under galvanized iron roof. Initial investigation conducted at the fire
scene revealed that fire started at right portion of the room of Edgardo Bandiola situated at the
third floor.

Bienvenida Bandiola, 85 years old, stated that “Habang nagsasampay ng damit sa third floor nakita ko na
umuusok sa loob ng kwarto ng anak ko. Kinuha ko ang hose sa may garden kaya lang hindi umabot sa loob ng
kwarto at hindi na mabasa ang sunog kaya bumaba na ako.”

Erick Leyco, 48 years old, neighbor of the fire victim, stated that “Nanunuod kami ng TV kasama ang
asawa ko ng may naamoy kaming sunog na kuryente. Bumaba ako ng bahay at nakita ko si manang Bidang na
sumisigaw ng “SUNOG”. Tinanong ko siya kung nasaan ang Breaker at pinatay ko yon at dumiretso na ako sa itaas.
Pagdating ko sa third floor ay sumalubong sa akin ang makapal na usok. Sinipa ko pa ang pinto ngunit malaki na
ang apoy na nasa may bintana kaya bumaba na ako at dinala ko na ang matanda sa labas.”

Ocular inspection conducted by the undersigned and Chief, IIU at the point of origin
revealed that it was heavily gutted including all the contents. No other adjacent establishment
and residential building were affected by this incident.

After declaration of fire out, the undersigned investigator cordoned/put yellow line at the
point of origin in order to preserve the area. Moreover, the undersigned personally handed
pertinent documents to the fire victim(s)/witnesses and requested them to appear before this
office to shed light on matters regarding said fire incident. Burned motor of an electric stand fan
and electrical wires were collected at the suspected focal point of fire eventually for laboratory
examination at Arson Laboratory Unit (ALU), BFP, National Headquarters located at Agham
Road, Brgy. Bagong Pag-asa, Quezon City.

No reported fatalities/injuries on the part of the fire victims and responding fire personnel
of this city.

DISPOSITION:

Progress Investigation Report will follow.
