Scenario Role Play (Health Education)

In Bekasi hospital, there is a patient who named Mrs. A. She just hospitalized for 1st
days ago at Srikandi’s room doctor said that he had medical diagnostic Pulmonary
Tuberculosis.

Now, nurse 1..will do the assessment to him

Nurse 1 : good moring Mr. A, How are you today?

Mrs. A : i’m not good nurse, i’m feeling breathless and nausea.

Nurse 1 : alright, how it feels?

Mrs.A : its really bad, i even couldn’t do my daily activity because im too weak to
move

Nurse 1 : have you ever felt like this before?

Mrs.A : no, I haven’t since I was a kid.

Nurse1 : Ok, is there any genetic disease that ur parent had? Such as diabetic,
heart disease, asthma, hypertension and etc?

Mrs.A : oh, I remember that my mother had asthma.

Nurse 1 : are you smoking maam?

Mrs.A : Yes, I am. For 2 packs per day and since I was 20 years old.

Nurse 1 : Ok maam, its enouogh for me, nurse 2 will check your vital signs. Are you
ok with that?

Mrs.A : yes sure.

Nurse 2 : okay mam i’ll check your blood pressure, pulse, respiration rate,and your
body temperature

Mrs. A : yes please

Nurse 2 : your blood pressure is 110/80 mmHg, its normal maam, your pulse and you
respiration rate little bit worried, they are 105x/minute and your respiration
rate is 24x/minute. That’s why you felt breathless. And now let me hear your
lung

Mrs. A : okay

Nurse 2 : take a deep breath... good.. once again, take a deep breath...okay that’s
good. Now, could you turn right maam?

Mrs. A : oh, okay

Nurse 2 : okay, take a deep breath.. good.. now i’ll clap your back, hmm i dont think
it’s good

Mrs. A : what wrong with me?

Nurse 2 : maam, doctor will explain it to you later.

Mrs. A :oh, okay

Nurse 2 :okay that’s all, if you need anything you can press the bell.

Then, the doctor is coming in to the srikandi’s room and explain to the patient about her
result assessment.

Doctor : good morning, i’m doctor that responsible to you

Mrs. A : oh morning doc

Doctor : according to your assessment we suspiciously that you had problem on your
lung, but to make it sure you have to do some test such as sputum test and
chest rontgen.

Mrs. A : okay

Doctor : okay, excuse me mam..

Nurse 3 : good morning mam.

Mrs. A : good morning nurse.

Nurse 3 : how do you fell now?

Mrs. A : i’m feeling the same as yesterday. Still feel breathless and nausea
Nurse 3 : okay mam, according the doctor said yesterday. Now we’ll take your
sputum and put in the tube. We’ll see if there is bacterial or not.

Mrs. A : but nurse i felt so hard to throuw it out the sputum. How to throuw it out,
nurse?

Nurse 3 : okay, my friend (Nurse 4) will demonstrate you to throuw your sputum with
take a deep breath and effective coughing.

Nurse 4 : hi, excuse me mam. Let me introduce my self, my name is .... you can
call me nurse.... now, i will demonstrate you to throuw your sputum. The
technique are take a deep breath and effective coughing. So, are you agree?

Mrs. A : yes, of course.

Nurse 4 : are your family here? Because, I will demonstrate not only with you but
with your family too in order to teach you after you back to home.

Mrs. A : yes, my mother is in the toilet. Wait for a minutes.

A few minutes later.

Mrs.A’s Mother : oh Nurse, what’s wrong with my daughter?

Nurse 4 : yes mam, I wil demonstrate you and your daughter about take a deep
breath and effective coughing to throuw her sputum.

Mra.A’s Mother : okay nurse. Do something well for my daughter.

Nurse 4 : of course. First, you have to prepare a glass of warm water.

Mrs. A’s Mother : okay, i will prepare it. This is a glass of warm water.

Nurse 4 : now, I will demonstrate you to take a deep breath.. first, sit or stand
with your elbows slightly back, after that inhale a deep breath through your
nose hold your breath for 3 seconds then exhale slowly and gently through
your lips for 4 or more count. Do it until three times, but at the third breath
cough it like this (nurse demonstrate of effective coughing) and put your
sputum in to the tube. Now lets do it together..

Nurse 4 : good maam, you can put your sputum in here, okay that’s good, after that
you can drink a glass of warm water to makes you feel better.
Mrs. A Mother : here is a glass of warm water

Nurse 4 : how it feels? Do you feel better?

Mrs. A : yes nurse because the sputum already spit out

Nurse 4 : okay then I’ll bring the tube and give it to analyst in laboratory.

Mrs. A Mother : thanks nurse 4, should my daughter to do it often?

Nurse 4 : Mrs. A can repeat it when she feels difficult to spit out sputum.

Mrs. A Mother: okay, then when we can see the result?

Nurse 4 : tomorrow you’ll get the result and then doctor will explain it to you

Mrs. A Mother and Mrs. A : thank you nurse

Nurse 4 : yes, excuse me ma’am

Tomorrow morning doctor and nurse 1 come to Mrs. A room to explain about the result of
sputum test

Nurse 1 : excuse me ma’am...

Mrs. A Mother : yes please... what will you do nurse?

Nurse 1 : this is doctor x, he will explain about the result of sputum test ma’am?

Mrs. A Mother : oh okay, so how about the result?

Doctor : according to the result, obtained that your daughter suffering of pulmonary
tuberculosis and I suggest to Mrs. A to get care for a week to stabilize her
condition and continuing the treatment ma’am

Mrs. A Mother : okay doc, will she get well soon?

Doctor : yes ma’am, but she have to routine to drink the medicine for 6 month.

Mrs. A Mother : thank you doc

Doctor : yes you’re welcome
