Subject: Power Slip for Rotary Slips (PSA-150)

Purpose: To inform the industry on the correct use of B+V Power Slip
for Rotary Slips

Effective: B+V PSA-150 (P/N 88210)

Description of incidents:

Blohm +Voss was informed of an incident with one Power Slip for Rotary Slips (air operated):

a) The driller engaged the rotary with the PSA-150 installed in the rotary table, the rotary spun with the
PSA and hit a man.

b) The crew locked the rotary table and used the PSA-150 as backup tong. Pins broke and the PSA-
150 got twisted.

Recommendation to the industry:

Do not spin with PSA-150 installed. Never lock the Rotary Table, free movement of the PSA-150 will prevent
from damaging (see picture 1).
Do not use PSA-150 as backup tong, the parts will be damaged.
