JOINT AFFIDAVIT

We, CHRISTOPHER B. MACUGAY and MARITESS Q. CAMPOS, both of

legal age, Filipinos, single (living together) and residing at Don C. Peralta, St.,
Brgy. 4, Laoag City, Ilocos Norte, after having been sworn in accordance with
law, hereby depose and state that:

1. We are the above-named affiants as well as the personal circumstances to

whom they pertain;

2. We have been living exclusively with each other, and represented

ourselves, as husband and wife without the benefit of marriage since
August 2010;

3. We were blessed with one daughter named DIENNE ASHLEY C.

MACUGAY;

4. We are both legally capacitated to marry each other and not barred by any
impediment to contract marriage;

5. Due to financial constraints as well as instability, and in light of the

pecuniary demand in meeting the needs of our daughter, we have not yet
contracted marriage up to the present;

6. When all the means are at our hands to shoulder all expenses necessary
and incidental to celebrate marriage, we hereby promise to marry each
other and fulfill all the obligations of marriage thereafter;

7. We execute this Joint Affidavit for the purpose of attesting to the truth of
all the foregoing facts and for whatever legal purpose it may serve.
