Instructions:
This form is to be filled by concerned student himself/herself who is intending to register course(s).
The student has to submit the form at concerned department’s relevant office before the deadline of registration. Late
submissions of registration forms will not be entertained.
Registration for improvement can be done for courses with F, W, D, D+, C, C-, and C+ grades only.
Student concerned will be responsible for any misinformation provided in this form.
A student may register for a maximum of 6 credit hours during Summer semester. During a regular semester
(Fall/Spring) a student may re-register for a maximum of 4 credit hours (in addition to the prescribed courses).
Student has to attach his/her latest transcript and original Department’s copy of fee payment receipt.
Separate registration form shall be used for registering courses at department other than the parent department, but
the form has to be submitted with the Semester Coordinator/CMS Operator of the parent department for onward
processing.
Registration fee once deposited is not refundable, adjustable or transferable.
