I, Omesh Viranjan De Silva of 185/12, Model Farm Road, Colombo 8

being a Buddhist do hereby state as follows:
1. I am the affirmant above named.
2. That I am one of the Directors of the two-member Board of

Directors of Academia Institute of Management (Pvt) Ltd
incorporated under the Companies Act No: 7 of 2007 bearing
registration No: 122199 having its registered office at No: 185/12,
Model Farm Road, Colombo 8.
3. That the Company, which was incorporated on 9th May 2007.
4. That the said Company did not commence any business since the
incorporation and neither did the Company open a Banking
Account.
5. That the Company has no assets and has not incurred any
liabilities.
6. As per Form 1 the Company had Five shareholders which included

the 2 Directors of the Company. But no investment in respect of
the shares have been made by any of the shareholders mentioned
in the said Form 1 and no share issue has been made.
7. That since the Company has no intentions of commencing any

business and/or transacting any business, it is appropriate that
the Company be struck off the Register under section 394 of the
Companies Act No: 7 of 2007.
8. That no prejudice will be caused to the public or the Government

of Sri Lanka by the decision to strike off the Company.
