Description of how to implement in the class: Students will first go through a regular lesson plan.
They will be introduced to the topic of graphing equations and how to manipulate those equations.
We will go through practice problems, worksheets and activities for additional practice, and a time
for students to take the mastery quiz through Microsoft Forms or to continue working on practice
problems and worksheets.

The classroom will have tablets or a computer cart for students to take quizzes after the completion
of their in-class worksheets and activities. Some students will finish their in-class assignments
quicker than others, and an immediate grade and feedback on their online quiz will be returned to
them on the screen. The teacher will have access to the results of all the quizzes taken on the
Microsoft Forms webpage.
Level of Technology Implementation (SAMR): On the SAMR model, Microsoft Forms can be used at
the level of Modification. It is a direct replacement for paper forms, and it also has the ability to give
results and feedback.
Description of the sample spreadsheet and graph/chart provided *: Please view the two pictures
below this form to see glimpses of how the Microsoft Form quiz will look like and what the teacher’s
results view will look like. The teacher’s response views are in the form of an overall pie-chart. The
teacher can also view individual submissions of every student. This is helpful so the teacher will
know if students need more practice or if they are ready to move on. As seen in the pictures below, a
of students missed number 2 on the quiz. This would be a signal to the teacher that he/she needs to
go over that information again since so many students missed it.
Other comments about your proposed spreadsheet/graph/chart activity: Microsoft Forms aims
to directly replace the traditional paper formed quizzes and exams. Although there are pro’s to taking
paper exams and quizzes, such as being easier to ensure that the student’s work is their own and not
someone else’s, the format of online forms in a great introduction to the modern world where most
all applications are online and give immediate feedback.
