PLEASE NOTICE ADDRESS HAS CHANGED!!!!

In consideration of the stated charge to be paid to carrier by this company, carrier agrees to expeditiously transport and deliver without
loss, damage, or delay. Carrier agrees to not back-solicit this company's customers. Furthermore, that the rate constitutes the full
compensation agreement and the carrier will not allege undercharge or attempt to recover any additional compensation from this company.
Carrier agrees to defend and hold harmless this company against any and all loss or damage claims on each shipment transported by
Carrier pursuant to this agreement. Carrier further agrees to defend and hold harmless this company from any and all liability, cost, and
damages to persons and/or property arising out of Carrier's operations hereunder, including but not limited to all road, fuel, and other
taxes, fees, or permits, related to the shipments transported by Carrier as arranged by this company as Broker.
The carrier shall issue a standard Bill of Lading to shipper showing shipper's load and count with seal numbers applied. The relationship
of the carrier to this company shall at all times be that of an independent contractor.

Any late delivery will result in a $200 penalty.

If late for more than five (5) hours the penalty will be $350 or 10% of the load amount. (whichever is greater)

introduced to Carrier by Broker Track Logistics Group, Inc. one (1) year following cancellation of this agreement.
