NURSING ASSESSMENT

Patient Charito Damian y Alarcon, 53 years old, Female, Filipino, and Widow with
five (5) children was committed in Metro Bacolod District Jail-Female Dorm last June 17,
2016 charged with the crime of Murder with Criminal Case No. 16-42603. She appears to
be medium in built, slouched posture and slow in movement; oriented to date, time, place
and people. She usually stays in bed and stares on object for a long period of time. She
dresses appropriately but unkempt and malodorous in terms of her grooming. When
being asked, she is cooperative and attentive but tends to cry most of the time. Her
emotional facial expression shows stressed and sad features.

Patient Damian speech manifests slowed rate, soft and stuttering voice, and
pauses long before answering questions. She usually verbalizes sadness, depression,
worriedness and avoids conversation to her fellow inmates.

She oftentimes expresses suicidal intentions because of her feeling of guilt and
hopelessness of her criminal case. She is paranoid most of the time to the people around
her that they might harm or kill her. As to her perception, she has auditory hallucinations
such as voice talking to her but she is not violent to her environment. She has awareness
of her illness but resistant to treatment. At present, she has no prescribed medications
taken.

NURSING ASSESSMENT

Patient Ma. Debbie Tuclaud y Lazarito, 38 years old, Female, Single with two (2)
dependents was committed at Metro Bacolod District Jail-Female Dorm last February 26,
2015 charged for the crime of Violation of RA 9165. Last August 14, 2015 the said inmate
had undergone a Psychiatric Evaulation conducted by Dr. Esther Regina B Servando,
Pscyhiatrist of Bacolod City Mental Health Center, Bacolod City and was diagnosed with
Bipolar Mood Disorder with Psychotic Features and Polysubstance Abuse Disorder. In
addition, these conditions are characterized by irritability, sexual indiscretions, auditory
hallucinations and use of prohibited drugs and/or alcohol repeatedly.

Patient Tuclaud is medium in built, has good eye contact and dressed
appropriately. However, her dorm mates complains of her dirty personal hygiene as
evident through her unkempt and malodorous grooming. She is irritable, talkative and
friendly at times. She get easily distracted when you’re trying to talk to her. She has no
remarkable distinguishing features.

She is not a violent person however, she cannot perform the responsibilities
assigned to her. She does not take a bath and does not observed proper personal
hygiene. She can’t stand longer conversation because of her short attention span.
