SUMMARY

Executive summary: This document contains the list of reports of investigation into
casualties, which have been reviewed by the Correspondence
Group on Casualty Analysis for this session of the Sub-Committee

List of reports circulated

1 The annex to this document contains the list of 38 reports of investigation into
casualties, which have been reviewed by the Correspondence Group on Casualty Analysis for
this session of the Sub-Committee. All the information related to the casualty and the analysis
of the reports of investigation can be accessed in the Global Integrated Shipping
Information System (GISIS) module on maritime casualties and incidents (MCI), which now
contains 9,926 incidents, 3,151 reports of investigation and 1,301 analyses.

Review by the relevant reporting States

2 The draft analyses of the reports have, first, been made available for review by the
relevant reporting Administrations only, using the secure access for Member States to GISIS,
for a 21-day period after notification by the Secretariat by email, in accordance with the agreed
procedure (FSI 17/20, paragraph 6.21).

3 Comments from reporting Administrations on the analyses of those reports of


investigation, which they have submitted to IMO, were collected by the Secretariat up
to 12 February 2016 and were processed in cooperation with members of the correspondence
group, according to their nature, i.e. either editorial or substantial.

Review by all Members of the Organization

4 After assessment by the reporting Administrations and receipt by the Secretariat of


confirmation that the analyses were representative of the significant factors of the reports, the
full text of analyses, which is available in GISIS, has been released, as indicated in
document III 2/INF.2, to all Members of the Organization for review by the Sub-Committee.
An appropriate number of copies of the draft analyses will be made available, upon request,
to the Working Group on Casualty Analysis, when established.

Implementation of chapter 14 of the Casualty Investigation Code

5 In support of the entry into force of the Casualty Investigation Code


on 1 January 2010, the Secretariat continued to manually enter available information on very
serious marine casualties into GISIS in order to facilitate the submission of full reports of
investigation into casualties and relevant annexes, in accordance with the Casualty
Investigation Code. The information recorded is intended to assist flag Administrations through
their consultation of the Dashboard contained in the GISIS module on reporting requirements.

6 The Sub-Committee is invited to note the information provided and to take action as
appropriate.
