Universal Design for Learning (UDL): This helps students with reading disabilities to be able to take their time
with answering questions. This is also a great tool for ELL students to be able to show that they do not
completely understand something without being isolated in front of the classroom.
Students will learn about the butterfly life cycle through different activities (see lesson plan on Beyond the
Basic Productivity Tools page) and then show their understanding through taking this quiz.
Describe the purpose of the SRT activity (check all that apply):
☐ Assess prior knowledge ☐ Anticipatory set (Create interest in a topic) ☐ To illuminate common
misconceptions ☒ Formative assessment of content knowledge (for purpose of differentiation and
mastery for ALL students) ☒ Summative assessment of content knowledge ☐ Test preparation
☐ Survey/Poll ☐ Discussion starter ☐ “Homework” collection ☐ Other (please explain):
Briefly describe what will happen DURING the SRT activity: Students will read the questions and then
show their cards with the correct answer while the teacher scans the room for the QR codes in order to
collect the data.
If you are unable to provide a working sample of your questions, please list them below (8-10):

Student Response and Assessment Tools
☒ Yes ☐ No
☐ Mixed (Some will have correct answers, other will not.)
Immediate corrective feedback: Will you pre-select correct answers to some of all of the questions and
display correct response to the class after the SRT activity?
☒ Yes
☐ No
Why or why not? In order to discuss and create better understanding, the answers will be given immediately.
Describe what will happen AFTER the SRT activity? After the activity the students will be able to see what
they got wrong and right and allow the teacher and students to discuss to create better understanding.
How will the data be used? The data will be used as an assessment grade for the students as well as the
teachers to collect data to understand what needs to be taught more in depth.
Describe your personal learning goal for this activity. From this lesson plan I am hoping students are able to
prove their understanding about the life cycle of a butterfly.
Reflective Practice: This could create a better understanding and allow for questions to immediately be
answered about the life cycle of a butterfly so that students are able to fully understand.
