I, LILY A. CRUZ, of legal age, Filipino citizen, and a resident of Block 58, Lot 15, Rosallina III
Village, Dumoy, Davao City, after having been duly sworn in accordance with law, do hereby
depose and state that:

1. I work as a Director of Human Resources under A. Bulan Trucking Services.

2. Rolando Suizo is under the employ and supervision of Amador Bulan and works as a
company driver.
3. Rolando Suizo, along with other drivers, was given daily operational briefings before
heading to their respective workstations.
4. The policies and protocols stated in the company handbook was duly observed.
