Extended Written Response Assessment

Unit: Fact or Opinion


Grade: 5th

Clear Purpose:
The purpose of this formative assessment is to offer insight to both the students and the teacher to
understand what we know about opinions, facts, and gathering information. This assessment is
designed to analyze what we need to know before we can move on to the next unit. Using scenario
written response and stand alone written response, both knowledge and reasoning skills will be
implicated to display student comprehension. After the test, students will have the opportunity to self
assess their level of preparedness. The teacher will have a one-on-one meeting with each student at
the end of the day and go over their self-assessment so that any frustrations or concerns can be
addressed quickly and positively, as well as encouragement given.

Directions: First, make sure your name is at the top of the paper. The test is to be taken quietly
and independently. When you are finished place your assessment in the “turn-in” file on my desk.
When you are finished you may choose to 1) read independently, or 2) work on creative writing and
drawing.

The first question involves a short story, or a “scenario” question. We will read the questions aloud
before you begin, then you will write your answer on the lines provided. If you are unsure of either
question please ask me.

The second question is a “stand alone question”. You will have to respond on the difference between
a fact and opinion on the lines provided.

Take your time on the questions; we have forty-five minutes to complete the assessment. If you
need more time, raise your hand and you can finish it in the study room.
Each question is worth 10 points, with a total of twenty points all together.
Section One: Scenario (10 points)

Below is box with a scenario. Take a side on the issue. State your opinion and support
it using your own knowledge and reasoning. Make sure you use complete sentences
and clearly state your answer.

Writing Situation: Teachers and educational experts say that video games is
rotting the mind of today’s youth. They maintain that young people spend too much
time staring at a screen. Do you think this is true? Do you think that children watch
too much television? Should children be restricted on the number of hours they play
video games? Write to parents and teachers explaining why or why not children
should have a restricted number of playing hours.
