Duties: Assisted with the development of CACREP Site Visit, teaching, research, and

Consultant- MERGE Counseling and Coaching, LLC- Orangeburg, South Carolina, October
13, 2012 and April 13, 2013.
Duties: Conducted enrichment seminars, such as study skills, note taking strategies, and time
management. Assisted in the management of dorm functions and creating a learning
environment for the high school students.
Counselor / Personal Development Teacher- South Carolina State Family Life Center-
Project FLAVA (Families Linked against Violent Activities)-Counseling Division-
Orangeburg, SC. 29118, August 18, 2005-2008.
Duties: Counseling at-risk students at Howard Middle School and Orangeburg Wilkinson
High School after-school program by conducting personal development and educatioanl
activities through motivational strategies. The workshops consisted of conflict resolution,
problem solving, goal setting, anger management, and team building activities. I received
Staff Volunteer of the Year Award for 2005.
Duties: Conducted educational and enrichment seminars, such as anger management, stress
management, and time management.
Duties: Professional counselor of disadvantaged adolescents that met the Medicaid eligibility
for services and the DSM-IV criteria for a mental health disorders; clients were treated with
individual psychotherapy, group counseling (i.e., anger, depression, and grief), family
therapy, and parenting assertiveness skills to help improve overall balance in social and
emotional functioning. Utilized the DAPE (Data, Assessment, Plan, and Evaluation) a system
of documentation with clients diagnosed with a mental illness or dual-diagnosis; provided
case management, progress notes and treatment plans consistent with Medicaid and Joint
Commission on Health Care Organizations (JCAHO) standards of care. Mental Health case
documentation consisted of an outpatient therapist daily activity report, multidisciplinary
integrated summary for treatment/service planning, treatment/service plan, general progress
interdisciplinary notes, progress note for individual and group, and Medicaid clinical finding
form.
Duties: Implementing lesson plans for the instructor, assisting with discipline concerns, and
helping to improve the overall development of the student. High Scope training facilitator
that addresses key childhood experiences in language, arts, science, and music.
Duties- Taught primarily with high school in Orangeburg County, South Carolina. I
taught literature, science, social studies, mathematics, history, and other subjects.
Implemented behavior management strategies to aid in learning for all students and
created lesson plans. Prepared classroom with needed supplies necessary to carry out
classroom activities. Kept track of individual student progress for class participation
during teacher’s absence.
Duties: Roving substitute teacher for elementary, middle, and high schools in Orange
County, Florida. I taught science, math, literature, science, study and test taking skills, and
job-readiness skills. Implemented and/or created lesson plans and managed classroom
procedures. Also, instructed students with learning and physical disabilities and behavioral
problems. Prepared, administrated and graded tests and assignments to evaluate students’
progress. Prepared materials and classrooms with learning activities. Kept student records
and tracked progress in long term assignments.
