 School Based Counselor- Lakeside Alternatives-Lockhart Middle School,

Duties: Professional counselor of disadvantaged adolescents that met the Medicaid eligibility
for services and the DSM-IV criteria for a mental health disorders; clients were treated with
individual psychotherapy, group counseling (i.e., anger, depression, and grief), family
therapy, and parenting assertiveness skills to help improve overall balance in social and
emotional functioning. Utilized the DAPE (Data, Assessment, Plan, and Evaluation) a system
of documentation with clients diagnosed with a mental illness or dual-diagnosis; provided
case management, progress notes and treatment plans consistent with Medicaid and Joint
Commission on Health Care Organizations (JCAHO) standards of care. Mental Health case
documentation consisted of an outpatient therapist daily activity report, multidisciplinary
integrated summary for treatment/service planning, treatment/service plan, general progress
interdisciplinary notes, progress note for individual and group, and Medicaid clinical finding
form.
