Student’s academic or behavioral area(s) of concern:


He is in the RTI process and has had many interventions done, and is not progressing. He is not
learning, not focusing, and is not engaged in lessons.

Summarize the activities during the observation (lecture, independent work, group work, etc.):
During the observation the children were working on writing worksheets and coloring
worksheets.

Based on the observation, summarize how the student’s area(s) of concern are impacting the
student’s performance in the classroom, both academically and behaviorally. (List how the
student participated with curriculum content/materials, group or individual instruction, teacher
interaction/direction, interaction with peers, or independent work, etc.):

The students were in groups at different tables working on their individual worksheets, and
conversing. Kyle was sitting on a chair that was able to rock back and forth and side to side.
The teacher reminded him to get working and he looked at her and then looked out the
window. For five minutes he was rolling on his chair, watching other children, moving his shoe
lace around, and did not look at his worksheet. The teacher again reminded him to get working
and then asked him if he wanted to sit at her table. He sat down at her table and read. The
teacher would point and he would read along while she pointed at the words. The teacher
then asked him how he would feel about the situation he read about. He then started writing,
and the teacher turned and helped another student. When the teacher was turned he stopped
writing and look around at the other children. The teacher asked the children to clean up, and
he asked if he had to finish the paper. She said that he would have to finish it later because
they were going to lunch. The children got in two lunch lines and he was in the back of the line
talking with another boy and laughing.
List any special occurrences (e.g. Fire drill, other student’s behavior, teacher behavior, substitute
teacher, other adults in classroom, special events etc.):

He is not progressing with the interventions used.

In order to get more academic information Kyle will need to be observed during instruction time.
