AFFIDAVIT OF LOSS

I, CHRISTIAN HARVEY N. WONG, of legal age, Filipino, with residence and

postal address at Poblacion, Libungan, Cotabato, after having been duly sworn to
in accordance with law hereby depose and state that:

1. I am presently employed with Landbank Davao Lending Center as an

Account Officer;
2. By virtue of my employment, I was directed to attend and travel via plane
for some bank-related activities in Cagayan de Oro City and Manila
between November 30, 2017 to December 5, 2017.
3. That upon my return to Davao City from Manila, I realized that one of my
Boarding Passes was missing,
4. Hereunder are the facts and circumstances surrounding the loss of my
Boarding Pass:

 That on December 3, 2017 at around 9:00am, I checked in for my

Cagayan de Oro to NAIA (MANILA) flight,
 That I used a CEBU PAC kiosk to check in, where it generated a thermal
paper boarding pass,
 I placed the boarding pass in my pocket and awaited for my flight.
 Upon arrival in NAIA, I proceeded to my temporary residence somewhere
in Malate, Manila to unpack my luggage including my boarding pass.
 That on December 5, 2017, after attending the AGFP seminar, I travelled
back to Davao City.
 That on December 8, 2017, I immediately prepared for my travel
liquidation of my recent official travels but were returned by the
Accounting Center due to a lacking boarding pass.

5. That, I made diligent search to locate the said Boarding Pass for the past
two (2) weeks but proved to be futile;
6. For all legal intents and purposes the said boarding pass is now
considered lost and beyond recovery;
7. I am executing this affidavit to attest to the truth of the foregoing and for
whatever legal purpose/s it may serve.

FURTHER AFFIANT SAYETH NAUGHT.
