First and foremost we would like to thank our Heavenly Father


for the continuous guidance and blessings. He has showed us from the beginning till the
culmination of this project, without him none of this world happens.

Secondly, we would like to say thank you to our parents for


their never – ending love and support not only financially but also emotionally, for
cheering us on when we are tired. You are a great inspiration.

For the members of the group who put 100 % of their effort
into making this project a reality, thank you guys. For patiently tallying, typing, writing
till we hours in the morning. It is greatly appreciated.

To our Clinical Instructor, Mrs. Leonida Rosales for her


unwavering support and guidance while we were in Barangay Sta. Rosa, Cainta, Rizal
and her continuous reminders while we were making this project.

To the Barangay Captain of Sta. Rosa, Cainta, Danilo San Juan


and his barangay officials for accepting us and allowing us to survey their community.

To the Rural Health Unit of Cainta Mrs. Rose Garcia, midwife


of barangay Sta. Rosa for the opportunity to practice our skills and to the BHW who
patiently support and guide us in surveying the community.

To our Dean Mrs. Arlene Blaise Cortez and our Level


Chairman Mrs. Yolanda Ortiz thank you for giving us the opportunity to experience this
and learn.
We would also like to thank the Bautista Family, for offering
their house to us whenever we need a place to stay while doing this project.

To the members of the community who are very


accommodating and warm in welcoming us to their homes, for their active participation
in our community survey, Thank you so much, without you, this project will not be
reality.

INTRODUCTION:

In every nursing students life must go through experiences that


may forever changes our lives. We may do things that we thought we couldn’t do but in
the end we learned we could. It was a rewarding experience because in one way or
another we know something will change in every one of us.

It was a challenge from the beginning, from the group to the


community, our patience was tested, and our social skills put in the line. Cooperation and
unity was very in demand virtue that time. If we need to finish with flying colors we must
stick together, there was no place for disappointment or misunderstandings. We need to
stay focused and strong at all times and set our differences aside.

When we started our survey of the community and saw the reality
of the situations of families living there, with no stable income to support their activities
of daily living. Something touched our hearts and we felt like it is not fair that some of us
complain of how little our allowances are when these families are struggling just to
survive one day at a time. We are blessed and we should be thankful for it and find way
share it with the less fortunate.

Community diagnosis is a very important tool to use for us to learn


the health situation of the community and in order for us to know the necessary measures,
solutions and program we need to implement to help improve its current situation. As
community health nurses we need to impart to the people what we know and mold them a
sense of well – being and the importance of being responsible for the health of their
families.
The community experience was very fruitful for us; it did not only
give us a chance to serve others but gave us a chance to know ourselves more. It made a
great impact in our lives for just a short period of time. After the experience we evolved
into stronger, more patient, more responsible human beings ready to handle whatever life
may bring our way. We changed and we are happy because we’re change for the better.

STATEMENT OF OBJECTIVES

GENERAL OBJECTIVE:

To be able to provide simple health services and to disseminate


health knowledge in the community of Barangay Sta. Rosa, Cainta Rizal in order to
increase their awareness regarding disease prevention and health promotion the
importance of taking care of their health.

SPECIFIC OBJECTIVES:

1. To be able to identify specific health problems that is present in the community.


2. To gather information and data from families surveyed the study.
3. To come up with attainable and suitable health care intervention this will improve
the health status of the members of the community as a whole.
4. To plan a program where people from the community can be involve and at the
same time will learn and eventually help them in their every day life.
5. To acquire and harness the necessary skills needed to be an effective community
health nurse.
SCOPE AND LIMITATIONS OF THE STUDY:

This study is limited to 50 families surveyed in Barangay Sta.


Rosa, Cainta Rizal. Where in information regarding their life style, socio – economic
status, and health related data were gathered and documented.

This was done through face to face interviews, observation and


questionnaire checklist with the specific families in order to have the most accurate and
reliable information possible.
SETTING OF THE COMMUNITY

DESCRIPTION OF SPOT MAP:

The community in Barangay Sta. Rosa, Cainta Rizal is one which


represents a mixture of people with different ethnicity, religious beliefs, and educational
background. Most of the residents are either Christians or Roman Catholic and is
reflected by the presence of their churches in the vicinity.

There is a balance between houses that are built with concrete,


wooden and mixed materials. A lot of houses are open with no fences except the well of
ones.

There are number of sari – sari stores in the community. These


stores are scattered all over the barangay, which is an advantage because it can cater the
needs of the community. There is also a few numbers of houses that caters native
delicacy like rice cakes and kutchinta and other pastries. There is a presence of
electricity, in general the community look clean except with some areas of the
community.

The whole barangay has a population of 2, 272. We were able to


survey the families who belong to P. Burgos Street, Barangay Sta. Rosa, Cainta Rizal.
SUMMARY AND CONCLUSION

Health is wealth. It is a gift from God and to take care of it is our


responsibility as human beings. Yet sometimes we take it for granted and we abuse it.
One of the biggest problems of every community, like Barangay Sta. Rosa is poverty. It
is the biggest culprit who rubs and deprives every Filipino people of a chance to have a
better quality of life and sometimes, even basic needs of food, shelter and clothing.

But although poverty reigns over the majority of the population we


must not lose hope because something can be done. First and foremost every member of
the community must start with themselves. For a change to happen it must first come true
within and soon it will have an effect on others then the community as a whole.

Cooperation is very essential to the success of every program


implemented in a community. No matter how good and beneficial the program is, without
the participation of the members it will not be a success. Leaders of the barangay must
motivate its members; show them the benefits of knowing how to take better care of their
health in order to avoid additional expenses of going to the doctor. Prevention is always
better than cure. Instill in them a sense of responsibility and discipline. And most of all
every leader must lead by example. Be a model to be followed, it will be great
contribution to your community and you will earn their respect.

Poverty must never be a hindrance to success we just need to act.


We cannot just sit down and wait for something that will change our situation in life.
Even the people winning the lotto and other games did something to win they joined.
They did not sit there and watch the joined. But we must not leave our future to luck; we
should find ways to improve our lives even if it means hard work. Laziness and
irresponsibility will not take you anywhere. Let your family be your inspiration and let
God be your guide and take the lead for every problem. He always have a solution and
never give up you in times of crisis.
