1. Application letter of proponent (RDMD-REMS F8)

2. Letter of application of the Dean/Director of the Institution where the student is
presently enrolled (Format attached) (RDMD-REMS F9)
3. Copy of thesis/dissertation proposal (text)
4. Timetable of the thesis/dissertation work.
5. Thesis/Dissertation Outline Approval sheet
6. Transcript of records and latest bio-data
7. Endorsement of thesis/dissertation adviser
8. Breakdown of budget not to exceed P 10,000 (MS) and P 15,000 (Ph.D.)
covering actual expenses to be incurred in the prosecution and reproduction
phases of the thesis/dissertation such as experimental materials, supplies,
typing, computer and binding services (with your signature affixed thereupon)
9. Certification that there is/is no other financial assistance from any other institution
in the thesis/dissertation
