Greetings of Peace and Prosperity!

I am Regodor V. Tabas, the owner of RVT Telecom and the current lessee of a
Public Market Stall in National Highway, San Geronimo, Bagabag, Nueva
Vizcaya. I am writing this letter to request for repairs of our rolling steel door.

Due to normal wear and tear since our occupancy in ___(year), our rolling steel
door has become difficult to close and open thereby adding risk to its further
damage and our store’s security. I hereby attest that such damage was not
deliberate on our part but simply due to its normal wear and tear for over a
period of __ years.
