APPLICATION PROCESS

□ 1. Fill out the Online Shifting/Transfer Application Form at http://cba.upd.edu.ph/.

□ 2. Complete and print the Shifting/Transfer Application Form (this form). Make sure the information on your online
application matches the information on your printed Application Form.

□ 3. Pay the P30.00 application fee at the Cashier’s Office (UPD Account No. 9682-500-499-450).

□ 4. On or before 19 June 2015, submit (a) the printed Application Form (this form); (b) application fee Official Receipt; and (c)
TCG/TOR issued by your college/unit (not required for VSB students shifting to/from BSBA/BSBAA).

□ 5. Wait for the results to be posted not later than 10 July 2015.
