7-202.11 - RESTRICTION
Observation: Fly pest strip found in the dough prep area.
Corrective Action(s): To prevent contamination, remove fly pest strip. Violation was corrected at the time of this
inspection by removing and discarding fly pest strip.
Code citation: (A) Only those poisonous or toxic materials that are required for the operation and maintenance of
a food establishment, such as for the cleaning and sanitizing of equipment and utensils and the control of insects
and rodents, shall be allowed in a food establishment.
(B) Paragraph (A), does not apply to packaged poisonous or toxic materials that are for retail sale.

4-202.18 - VENTILATION HOOD SYSTEMS FILTERS
Observation: Pizza oven exhaust hood vent found misaligned.
Corrective Action(s): To provide for proper ventilation, reset pizza oven exhaust hood filter.
Code citation: Filters or other grease extracting EQUIPMENT shall be designed to be readily removable for
cleaning and replacement if not designed to be cleaned in place.
4-602.13 - NONFOOD CONTACT SURFACES
Observation: Pizza oven light shields found with a build-up of grease and dust.
Corrective Action(s): To prevent contamination, clean all equipment on a routine basis to prevent accumulation of
soil residues.
Code citation: Nonfood CONTACT SURFACES of EQUIPMENT shall be cleaned at a frequency necessary to
preclude accumulation of soil residues.
4-602.13 - NONFOOD CONTACT SURFACES
Observation: Ceiling vent at 3-compartment sink area was found with a build-up of food debris.
Corrective Action(s): To prevent contamination, clean ceiling vent.
Code citation: Nonfood CONTACT SURFACES of EQUIPMENT shall be cleaned at a frequency necessary to
preclude accumulation of soil residues.
6-501.12 - CLEANING, FREQUENCY AND RESTRICTIONS
Observation: Food and food debris was found on floors beneath cabinets/shelves in prep area.
Corrective Action(s): to prevent contamination, clean the physical facilities at a frequency necessary to keep them
clean.
Code citation: (A) The physical facilities shall be cleaned as often as necessary to keep them clean.
(B) Except for cleaning that is necessary due to a spill or accident, cleaning shall be done during periods when the
least amount of food is exposed such as after closing.
6-501.14 - CLEANING VENTILATION SYSTEMS, NUISANCE, AND DISCHARGE PROHIBITION
REPEAT OBSERVATION Pizza oven exhaust hood filters were found with a build-up of grease and dust.
Corrective Action(s): Clean intake exhaust air ducts, exhaust air ducts and filters so they cannot be a source of
contamination.
Code citation: (A) Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of
contamination by dust, dirt, and other materials.
(B) If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful
discharge.
