Student: Allison

Allison is an 8 yr old who enjoys the company of her peers but has a hard time initiating
friendships, she spends 25% of her time in a kindergarten class and the rest of her time in a
resource room with one on one instruction. She has attended regular preschool and
kindergarten. She has been categorized in the autism spectrum but has yet to be diagnose.
Alison can hold basic conversations but has trouble with cognitive parts of learning,
identification of and similarities and differences of numbers and letters. Alison can identify
basic colors, some secondary colors and shapes, she is unable to consistently recall numbers
and letters. Her parents request that increasing her social abilities and relationships are their
top priority.

Goals: Each goal needs to be filled out completely with each goal being written in SMART form.
Please complete all 3 goals based on Allison’s needs.

Goal 1:

Continue a conversation through multiple exchanges.

Present Level of Performance:


Although Alison can engage in conversation with her peers once started for her, she struggles with
initiating conversations for herself.

Annual Goal:
Alison will initiate a conversation with her peers when the opportunity presents its self and hold the
conversation for the length of three volleys/ back and forth questioning by herself by the next
evaluation. For consecutive observation by next review.

Evaluation Procedure/Assessment Method


Alison will start a conversation with her peers and ask them questions and answer their questions.

Benchmark 1:
Alison will have started 2 different conversations a day with 2 different peers, and asking 2 questions
and answering 2 of their questions by the end of the first trimester.
Benchmark 2:
Alison will have started three conversations a day with three different peers asking three different
questions and answering three of their questions by the end of the second trimester.

Demonstrate basic knowledge of one-to-one letter-sound correspondences by producing the primary

Can not consistently recognize letters.

Annual Goal:
Letter and sound identification of all 26 letters in the alphabet by the end of the year

Evaluation Procedure/Assessment Method


1 hour small group instruction 5 days a week for the duration of the year.

Benchmark 1:
By the end of trimester one Alison will recognize 25% of the letters with 15% of sounds.

Benchmark 2:
By end of Trimester two Alison will recognize 50% of letters with 30% of sounds.

With prompting and support, retell familiar stories, including key details

Present Level of Performance:


Can identify pictures appropriate for younger children but struggles with abstract, higher order or
levels of representation.

Annual Goal:
Identify abstract, high order, different levels of representation, such as emotions or places and
seasons by the end of the year.

Evaluation Procedure/Assessment Method


Small group teaching of subject matter 30 minutes a day 5 days a week for the duration of the year.

Benchmark 1:
Alison will be able to identify 20% of grade level evaluation by end of first trimester

Benchmark 2:
Alison will be able to identify 40% of grade level evaluation by the end of the second trimester.

Reading identification: 15 min identifying and learning abstract/higher order/ different levels of
representation, 15 min activity: color page, story out loud etc.
Writing/ Letter identification each day for 30 min followed by a 30-min activity to assist with
memorization.
Communication, class time spent with her peers to allow for normal interactions and appropriate
conversation starters.
