BEFORE HOSPITALIZATION DURING HOSPITALIZATION
HEALTH PERCEPTION “Importante ang kalusugan kahit
saan mo tignan” as verbalized by
the patient. He describes himself
as a healthy and strong individual
capable of performing his
activities of daily living. He
manages his health by taking a
bath daily.
NUTRITIONAL METABOLIC Patient RS eats nutritious food;
PATTERN his meal is usually 2-3 plates and
more on vegetables, fish and
meat. He usually consumes 2-3
litres of water a day. He only
drinks alcoholic beverages
occasionally. He has no allergy
to food and medication. He
doesn’t have any difficulty in
swallowing both liquids and
solids and doesn’t use any food
supplements as he has a good
appetite as according to him.
ELIMINATION PATTERN He usually voids 8-10 times a
day, each void with pale yellow
colored urine; he defecates 2
times a day with formed brown
stool, he has no difficulty in
defecating.
ACTIVITY- EXERCISE Patient RS goes to farm everyday
PATTERN and go home after. He performs
self-care independently and
walks everyday going to his farm
as his exercise.
SLEEP-REST PATTERN Patient RS said that he usually
sleeps for about 7 hours a day, at
night he sleeps from 10:00 pm
and wakes up 5:00am. He also
added that he doesn’t use any
sleeping aids as he easily goes to
sleep.
COGNITIVE-PERCEPTUAL Patient RS is an undergraduate,
PATTERN he only attained 3rd year college.
He can speak Iloco and Tagalog.
He can read and write. He could
also recall events readily. He
doesn’t have any deficits on
hearing, touch, smell and taste.
SELF-PERCEPTION PATTERN Patient RS is a responsible
husband and father. He works
hard for them. He also helps his
wife do household chores if he
has time. He also jams with his
friends during his free time.
ROLE RELATIONSHIP Patient RS is a farmer. He lives
PATTERN with his wife and daughter. And
according to him, he is a caring
and a loving father and a good
husband to his wife.
SEXUALITY-PRODUCTIVE Patient RS was circumcised
Patient RS follows the doctor’s
and nurses’ advices as he
anticipates that the
services/advices are due for his
early recovery.
According to patient RS, he only
consume 1 ½ cup of rice with
fish and chicken. He consumes
1L of water and he also drinks
pineapple juice during his
hospitalization.
He voids 3-5 times a day, he
defecates every other day due to
his new environment.
Patient RS cannot continue his
usual activities of daily living
because of his condition. He
needs assistance when going to
the bathroom, to eat his meal and
to sit on the bed.
He has difficulty in sleeping due
to his new environment. He
sleeps with a duration of 1-2
hours.
Patient makes eye contact during
conversations and expresses his
feelings that correspond to the
situation. He responds to
question with interval of 5-10
seconds. He is also aware of his
current condition.

by the patient.
Since he was hospitalized, he
couldn’t go to work and cannot
accomplish his tasks due to his
condition. He was still happy for
the reason that he appreciates the
presence and support of his wife
and daughter.
He has no sexual activity.

COPING-STRESS According to patient RS, He comforts himself by sleeping
