Interpersonal Communication Process Recording

General Objectives:

• To establish a nurse-client relationship.

• To guide the client toward identifying a plan of action to a satisfying and socially acceptable resolution.

Specific Objectives:

• To establish nurse-patient relationship.

• To maintained trust and rapport with the client.

• To facilitate the clients expression of thoughts and feelings.

• To promote and improve therapeutic communication.


Evaluation:
This was our third day at CCMH but our second day to handle and mingle with our patient. We get
the patient from the ward and let them to brush teeth and wash their face. I’ve noticed that my patient
was so excited for the new activity. When he hear about the play therapy, I see to his face the smile and
excitement and I asked to him what comes to his mind when I say play therapy and he said that he think
of prizes.

During the play therapy that we conducted, He was so active and often participated to different
games. A few minutes later, upon observing my client, I noticed that my patient seem to be lost in his own
thoughts and not paying attention to the one who are speaking in the front. At this situation, it is important
to allow the patient to have sufficient time to concentrate or respond even it seems like a long time.

For the third day that I met my patient after our conversation, there’s an eager feeling that I want to
be part of his healing process. I put myself on him, what if I am on his situation. I hope that during my duty
on CCMH with V.G., I can see the progress on him becoming good and lessen the onset of schizophrenia.
It’s a stressful day, but a productive and happy day also. I hope that I can learn more from V.G. and also
from the other patient of CCMH.
