5. Is a pre-stowage plan prepared and delivered to agents / stevedores at load port? If yes, please
attach a copy.
6. Are cargo holds properly rinsed with fresh water before loading?
7. Are cargo holds and bilges properly cleaned and dried before loading?
8. Are the hatch covers properly maintained and are following items checked and in order?
a. Hatch cover panels (state type)
b. Sealing rubbers and their retaining channels
c. Compression bars
d. Water (drain) channels and their gutter sheets
e. Non-return valves on drain channels (hatch coaming corners)
f. Cross joint cleats and wedges
g. Quick acting cleats and rubber washers
h. Open / close system (state type)
i. Ventilation system and covers
9. Is any type of hatch cover test (ultrasonic, hose test, chalk test, light test) carried out by
shippers / charterers / owners before commencement of loading? State type of test, date / time,
testing party and result, attach report if available.
13. How is the cargo transported from warehouse / storage alongside the vessel? State type and
details i.e. trucks, wagons, special carrier, covered with tarpaulin, wet during transportation etc.
15. Are proper slings used for loading the cargo? State type of slings. If not proper, please state
type of damage caused by wrong sling usage.
16. Are forklifts used for stowage in holds? If yes, please state whether the forklift blades were
properly protected in order not to damage the cargo.
17. Are proper and enough dunnage and chocking material used by stevedores?
18. Please state all stevedoring damages to the cargo, which occurred during loading operation.
19. Is the loading, stowage, separation and lashing made according to the pre-stowage plan and
cargo securing manual? Please state type of lashing.
20. Were there rainy / snowy periods during the loading operation? Please state date and time.
Were hatch covers closed timely? Were hatch covers properly drained before opening in order to
avoid dropping into holds?
21. Please state any other remark / opinion you may have regarding the cargo condition and
loading operation.
23. Please check the Bs/L and M/Rs for following details:
a. Are they correctly dated?
b. Are load port and discharge ports correct?
c. Is the cargo description correct?
d. Is the cargo quantity and weight correct? Please state whether tally and
or draft survey has been made by crew.
24. Please list your remarks inserted into Bs/L and M/Rs reflecting the apparent damages on the
cargo. If more then one B/L and / or M/R issued, please attach copies.
25. Please state whether “Clean on Board” wording was deleted from the Bs/L and / or M/Rs, if
they are claused with remarks.
26. Please state whether you have received specific ventilation instructions from the shippers? If
not, please ensure that the common ventilation procedure will be followed during voyage.
