QUICK GUIDE

SUBMIT CERTIFICATE OF SERVICE (COS)

Enter UIS credentials (username and password) > Login button

UP Employee Self Service responsibility > Employee Submissions/


Requests folder > Certificate of Service folder > Submit COS
3 Submit Certificate of Service: Special Information

Add button
4 Certificate of Service

Enter details: Position*, Start Date*, End Date*, Activity/ies, and


Number of Hours > Apply button

*Required fields

To fill out the Position field, click the Search button (magnifying
glass). On the pop-up window that will open, click the Go button to
display the list of values.
5 Submit Certificate of Service: Special Information

Select COS with ‘New’ status > Next button


6 Submit Public Service: Review

Review details entered, including Approvers > Add attachments


(optional/ if applicable) > Submit button
A confirmation message will appear that the COS has been forwarded
for approval.
SUBMIT CERTIFICATE OF SERVICE

Step 1. Log in to uis.up.edu.ph

Step 2. On the UIS Main Menu, click the UP Employee Self Service
Responsibility.

A dropdown list will appear.


Step 3. Select the Employee Submissions/ Requests folder.

Another dropdown list will appear.

Step 4. Click the Certificate of Service folder.


Step 5. Select Submit COS.

You will be directed to the Submit COS: Special Information page.

Step 6. Click the Add button.

You will be directed to the Certificate of Service page.


Step 7. Fill out the fields: Start Date*, End Date*, Position, Activity/ies other
than Teaching, Number of Hours, and Except (Absence Date).

Step 8. Click the Apply button.

You will be redirected to the Submit COS: Special Information page.


Step 9. Select the entry with ‘New’ status, then click the Next button.

You will be directed to the Submit COS: Review page.


Step 10. Review the details that you entered and the approvers of your
COS listed under the Approvers subsection.
Note: If the names of approvers listed are incorrect or out-of-date, kindly contact the
HRDO to request for the updating of your record. Approvers include the immediate
supervisor and/ up to the Principal Unit (e.g. Dean).
Step 11. If there are no more changes, click the Submit button.

A confirmation that your COS has been forwarded to your supervisor


for approval will appear.

Note: COS may be printed only after it has been approved by the final approver.

APPROVE COS

Note: COS will be approved by the immediate supervisor and/ up to


the Principal Unit.

APPROVE CERTIFICATE OF SERVICE

Step 1. Log in to uis.up.edu.ph

Step 2. On your UIS Worklist, click the item with the Subject “Certificate of
Service for [Name of Employee] requires approval.
Step 3. Click the Approve button.

Note: If you are the final approver, the employee will receive a notification on his/ her
worklist. If you are not yet the final approver, the request will be forwarded to the final
approver’s worklist.

Other Approver Actions

APPROVE

Click the Approve button if you accept a completed and verified transaction or submission.
Once approved, it will be automatically forwarded to the worklist of the next approver, or if
you are the final approver of the transaction, the originator/ requester will be notified of the
status.

RETURN FOR CORRECTION

Click the Return for Correction button if some details on the submission need to be
corrected or changed by the requester/ originator. The submission will be then sent back to
the requester/ originator
REASSIGN

There are two options in reassigning a transaction to another approver: Delegate (Forward)
or Transfer.

When you select Delegate (Forward), you give another user or approver the authority to
respond to the notification on your behalf. This option allows you (the original approver) to
retain the ownership of the notification as well.

Transfer, on the other hand, entails the change of not only the authority to approve, but also
of the ownership of the notification to another user or approver.

REJECT

Choose the Reject button if you disapprove of the transaction. Note that once you click
Reject, the submission or transaction and the details it contains shall be discarded.

PRINT CERTIFICATE OF SERVICE (COS)

Note: COS may only be printed after it has been approved.


A notification will be received in the UIS Worklist (right side of the UIS
Home page) re approval.

3 Schedule Request: Parameters

Set the parameters: Position*, Start Date*, and End Date* > Next
button

To fill out the Position field, click the Search button (magnifying
glass). On the pop-up window that will open, click the Go button to
display the list of values.
4 Schedule Request: Layout

Next button
5 Information

Take note of the Request ID > OK button


6 Requests

Request ID > corresponding Output button > Print the output

Note: If the Output button does not appear right away, click the
Refresh button located above the Request ID column.
7 Submit hard copy of COS to HRDO
PRINT CERTIFICATE OF SERVICE

Note: COS may only be printed after it has been approved by the final approver.

Step 1. Log in to uis.up.edu.ph

Step 2. On your UIS Worklist, check if there is a notification that the final
approver has approved your COS.
Step 3. On the UIS Main Menu, click the UP Employee Self Service
Responsibility.

A dropdown list will appear.

Step 4. Select the Employee Submissions/ Requests folder.

Another dropdown list will appear.


Step 5. Click the Certificate of Service folder.

Step 6. Select Print COS.

You will be directed to the Schedule Request: Parameters page.


Step 7. Fill out the parameters: Position, Start Date, and End Date, then click
the Next button.

You will be directed to the Schedule Request: Layout page.

Step 8. Click Next.

You will be directed to the Schedule Request: Review page.


Step 9. Click Submit.

Step 10. Take note of the Request ID, then click OK.

You will be directed to the Requests page.


Step 11. Click the corresponding Output button of your Request ID.

Note: If the Output button does not appear right away, just click the Refresh button
located above the Request ID column.

You may now print and download (in PDF) your COS.

Step 12. Submit printed copy of COS to Human Resources Development Office
(HRDO).
