Section A. Client Support Profile

Each Provider provides Texas Pregnancy Care Network with a sample Client Support Profile, which is a
story that demonstrates some of the work of the Texas Alternative to Abortion Services Program with
one particular client. The Client Support Profile is not an inclusive summary of all services that a client
receives from a Provider. All Client Support Profile stories are used with the written permission of the
client, and the client’s name is changed to protect the client’s identity.

We found Suzie waiting for us outside the office as we pulled into the parking lot to begin the day. She
could not wait for us to open the door and turn on the lights. She was there for a free pregnancy test
to confirm the home tests she had taken. Her nervousness was apparent as she completed her intake
form. We offered her refreshments but she refused. She just wanted to take the test.

A staff member sat with Suzie and began a counseling session. Married for 13 years, she was thrilled
at the possibility that she might be expecting, but there was something more in her demeanor. They
talked further and the staff member asked her questions about her relationship, job, goals and tried to
put her at ease.

Then Suzie burst open. Her complex relationship unfolded. She and her husband had drifted apart
over the years and she had strayed. After she told her husband everything, he was hurt and angry
and began an affair as well. Meticulous, Suzie kept a diary/calendar, but she still did not know who the
father might be. She was hoping the pregnancy test would reveal some clue.

After years of praying for a child, Suzie was considering abortion. Hours later the staff member and
Suzie talked more about relationships, mental and maternal well-being, fetal development and the
enriching journey for which she had longed for. Suzie seemed more at ease and agreed to the free
ultrasound. She even wanted to bring her husband and revealed that it was he who had prayed
alongside with her for a child for so many years.

A few days later, Suzie and her husband returned for the free ultrasound. Results in hand, the date
confirmed that her husband was indeed the father of the baby, the timeframe they both knew to be
true. After more counseling, the couple was determined to work together in support of one another
and the new life Suzie is carrying. They continue to drop by the office to say “hello” and “thank you.”
Section B. Facilities Tour

• Comments: The facilities, including exterior, client waiting areas, floor plan, safety features,
counseling rooms, storage rooms, and classrooms were in substantial compliance with
Program requirements.

• Discrepancies: None

• Corrective Action Recommended/Taken: None needed

Section C. Program Administration

• Comments: Program administration, including the storage of client records, storage of client
education materials, policies regarding background checks, locations of policy and procedure
manuals, separation of spiritual materials, and posting of confidentiality, grievance and non-
discrimination notices were in substantial compliance with Program requirements, except as
noted below.

• Discrepancies:
1. One or more Program notice(s) was not in substantial compliance with Program
requirements.
2. One or more educational materials were not in compliance with Program rules relating to
storage of client educational materials.
3. Access to client files needed to be better secured.

• Corrective Action Recommended/Taken:


1. Provider revised and verified that Program notice(s) were in substantial
compliance with Program requirements.
2. One or more educational materials were not in compliance with Program rules relating to
storage of client educational materials.

Section D. Required Records

• Comments: Required program records, including floor plans, policies and procedures,
organizational charts, certifications, employee and/or volunteer manuals, and training
materials were made available and were in substantial compliance with Program requirements,
except as noted below.

Section E. Review of Reimbursement Requirements


• Comments: Texas Pregnancy Care Network reviews billing compliance on an ongoing basis
throughout the year. A report of client services that have been negated and recouped will be
included in the Quarterly Report for the 4th Quarter.

Section F. Summary of Report and Provider Client Services

The annual monitoring of the Provider resulted in corrective actions listed for each discrepancy.

Foundation for Life has been in existence since December 1976. The Maternal Assistance program
provides confidential counseling, resources and other essential items for women in crisis pregnancy
situations, including free education classes, workshops and baby and maternity items. This
organization serves the Harris County community, as well as Jefferson, Fort Bend, Brazoria, Liberty,
Victoria and Galveston counties.
