AFFIDAVIT OF LOSS
I, Mr. Reynaldo Beloy Jr., of Legal Age, Filipino, Single, with residential address at Zamora
St. Tacloban, City and with Cebu, City address at Dorm 1 UCMETC, Alumnus Mambaling, Cebu ,City
subscribing under Oath, hereby depose and state:

1. That I am the holder of a valid School Identification Card issued by the University of Cebu-
Maritime Education and Training Center;

2. That I am a duly student of UC-METC as Bachelor of Science in Marine Transportation;

3. That Last March 4, 2018, I placed the aforesaid school identification Card inside my wallet
while I was at the down town area of Cebu, City;

4. That when I looked for my school Identification Card inside my wallet, I could no longer
locate it;

5. That my school identification card could no longer be found notwithstanding diligent

efforts to locate the same and is now considered lost for all legal intents and purposes;

6. That I certify under this oath to report the loss of my school identification card issued by
the University of Cebu-METC to avoid the said missing identification card from being used
for illegal purposes by other persons;

7. That I execute this affidavit to attest the foregoing truth of all the foregoing facts.

Affiant further says none.
