Section A. Client Support Profile

The following paragraph describes a typical case successfully managed by the Provider prior to its enrollment
in the Texas Alternative to Abortion Services Program. It is shared for the limited purpose of illustrating the
quality and scope of the Provider’s work prior to its participation in the Program. The clients have given
their consent for their story to be included in this report, and their names have been changed to protect
their privacy. The services described in the Client Support Profile were not provided with Program funds.

The organization received a call this past summer from a client who just wanted to say thank you – 10 years
later. As an abortion-minded client, the counselors worked with her, took her to doctor’s appointments, got
her food, clothing, plus all the items her little one would need when he was born and afterwards in the
following months. She “never forgot the kindness and the love she felt.” “You made a difference in my life,”
she added.

Section B. Facilities Tour

• Comments: The facilities, including exterior, client waiting areas, floor plan, safety features,
counseling rooms, storage rooms and classrooms were in substantial compliance with Program
requirements.

• Discrepancies: A fire extinguisher and a “No Smoking” sign were missing. Also, illuminated exit
signs and smoke detectors were missing throughout the location.

• Corrective Action Recommended/Taken: A fire extinguisher was purchased, a “No Smoking” was
posted and illuminated exit signs and smoke detectors were placed throughout the building.

Section C. Program Administration

• Comments: Program administration, including the storage of client records, storage of client
education materials, policies regarding background checks, locations of policy and procedure
manuals, separation of spiritual materials, and posting of confidentiality, grievance and non-
discrimination notices were in substantial compliance with Program requirements.

• Discrepancies: Spiritual materials were not labeled and separated from the non-spiritual materials.
Also, a Nondiscrimination, Confidentiality and Grievance Notices were not posted.

• Corrective Action Recommended/Taken: Spiritual materials were labeled and separated from the
non-spiritual materials. Nondiscrimination, Confidentiality and Grievance Notices were posted.

Section D. Required Records

• Comments: Required program records, including floor plans, policies and procedures, organizational
charts, certifications, employee and/or volunteer manuals, and training materials were made
available and were in substantial compliance with Program requirements.

• Discrepancies: Copies of the Client Services Policies and Procedures manual were missing in areas
easily accessible to staff and volunteers.

• Corrective Action Recommended/Taken: Copies of the Client Services Policies and Procedures
manual were made available in areas easily accessible to staff and volunteers.

Section E. Summary of Report and Provider Client Services

Foundation for Life has been in existence since December 1976. The Maternal Assistance Program provides
confidential counseling, resources and other essential items for women in crisis pregnancy situations,
including free education classes, workshops and baby and maternity items. This organization serves the
Harris county community, as well as, Jefferson, Fort Bend, Brazoria, Liberty, Victoria, and Galveston
counties. Family Promise of Lubbock also services clients nationwide. The organization receives referrals
from social service, medical providers and government agencies. The organization makes referrals most
often to government and social service agencies and medical providers.
