Greetings!

The Samahan ng mga Detinado ng Tarlac Provincial Jail (SDTPJ) is

requesting for the approval of the headcounting of male inmates, which will be done
around the platform of the Bulwagan during 12:00 o’clock and 3:00 o’clock
headcount. This will ensure that those elderly and those who has illnesses such as
hypertension will not be compromised.

SUBJECT : Request for Medical Officers for Medical Mission

DATE : November 24, 2016

1. Reference: Weekly medical check-up of inmates by Dr Raymund Talon of

Talon General Hospital on November 14 and 21, 2016.

2. Some inmates are suffering from illnesses such as diabetes, hypertension,

bleeding disorder, cough and colds, and uterine problems.

3. In view of the foregoing, may we request for medical doctors particularly

OB-Gynecologist and internal medicine doctors preferably on Mondays,
Wednesdays, and Fridays as scheduled non-visiting days at Tarlac Provincial Jail.
