NORMAL REGISTRATION

Normal birth registration application means every birth occurring in Malaysia should be registered within 14
days from the date of birth of the child.

[Section 8, Act 299]

APPLICATION PROCEDURES

Birth registration application can be made at any NRD Office throughout Peninsular Malaysia by submitting
the completed Form JPN.LM01 and attaching supporting documents.

SUPPORTING DOCUMENTS

1. Personal Identification document or valid travel document (if mother/father/informant is not a


citizen).
2. Expectant mother examination document
3. Confirmation of birth or stillbirth
4. Marriage document of parents (for first child only) – if married

No fee is imposed.

LATE REGISTRATION

Late birth registration application means every birth occurring in Malaysia which is registered after 42 days
from the date of birth of the child.

[Section 12(1), Act 299]

APPLICATION PROCEDURES

Late birth registration application can be made at any NRD Office throughout Peninsular Malaysia by
submitting the completed Form JPN.LM01 and attaching the supporting documents. Each application made
will be accepted after the result of the search is obtained by using form JPN.LM14 (except birth which
occurs after 2003).

1. Personal identification document or valid travel documents (if mother/father/informant is not a citizen)
2. Expectant mother examination card/book (if any)
3. Birth or stillbirth confirmation form (if any)

DELAYED REGISTRATION

Delayed birth registration application means registration that is made within 15 to 42 days from the date of
birth of the child and the birth occurs in Malaysia.

[Section 11(1), Act 299]

APPLICATION PROCEDURES

Birth registration application can be made at any NRD Office throughout Peninsular Malaysia by submitting
the completed Form JPN.LM01 and attaching the supporting documents.

SUPPORTING DOCUMENTS

1. Personal identification document or valid travel documents (if mother/father/informant is not a


citizen).

3. Confirmation of birth or stillbirth

REGISTRATION WHILE ON PILGRIMAGE

Each birth which occurs while on pilgrimage should be reported to the registrar in Hedjaz within 40 days
from the date of birth or reported to the NRD Headquarters counter within 6 months from the date of birth.
However, approval will only be given by the Registrar-General at NRD Headquarters.

APPLICATION PROCEDURES

Hedjaz birth registration application can be made at any NRD Office throughout Peninsular Malaysia by
submitting the completed Form JPN.LM01 and attaching the supporting documents.

3. Personal Identification Document of Informant (if the informant is not the parents)

FEES

No fee is imposed.
RE-REGISTRATION (MISSING & DESTROYED)

Birth re-registration application is registration that is made because the register or the record of the birth
cannot be traced, probably missing or destroyed, by submitting proof that the birth has been registered.

[Section 4A, Act 299]

APPLICATION PROCEDURES

Applicants need to complete form JPN.LM01 and attach the supporting documents. Birth registration can be
made any NRD Office throughout Peninsular Malaysia.

SUPPORTING DOCUMENTS

1. Original Certificate of Birth (if any)


2. Family Information Form – BMK81
3. Extract of School Register
4. Copy of Identity Card of the Child (if any)
5. Any document with birth registration number for re-registration of birth

FEES

No fee is imposed.

RE-REGISTRATION OF LEGITIMATED PERSONS

An application for the re-registration of birth of a legitimated person is a re-registration of the birth of an
illegitimate non-Muslim child who has been registered and then legitimated.

[Section 17, Act 299]

APPLICATION PROCEDURES

Applicants need to complete form JPN.LM01 and attach the supporting documents. Birth registration can be
made any NRD Office throughout Peninsular Malaysia.

SUPPORTING DOCUMENTS

1. Original Certificate of Birth


2. Identity Card of the Child (if any)
3. Marriage Certificate of Parents
4. Court Order that the child has been legitimated
5. Identity cards of mother and father

FEES

Application fee:

Free if the registration is made within 3 months from the date of marriage of parents

Fee of RM 30.00 is imposed after 3 months from the date of marriage of parents
ALTERING NAME OF CHILD

Application to alter the name of a child which has been registered can only be made within 1 year from the
date of birth of the child. However, the name of a child that has been registered CANNOT BE ALTERED if
the child has attained the age of 1 year or more from the date of submission of application.

ENTERING NAME OF CHILD

i. APPLICATION TO ENTER NAME OF CHILD UNDER 1 YEAR OLD

Application to enter the name of a child below 1 year old who has been registered as “UNNAMED”.

Application to enter name of a child aged 1 year but below 21 years who has been registered as
“UNNAMED”.

CORRECTING ERROR OF INFORMATION

i. APPLICATION TO CORRECT CLERICAL ERROR

Application to correct minor error which only involves spelling error which if corrected does not alter the fact
in the Birth Certificate.

Applicants need to complete form JPN.LM21 and attach the supporting documents.

ii. APPLICATION TO CORRECT ERROR OF FACT

Application to correct error of fact where the information other than the name of the child provided by the
informant during the registration of the birth is incorrect or not available. The applicant needs to submit
supporting documents to prove the error of fact.

APPLICATION PROCEDURES

Applicants need to complete forms JPN.LM21 and JPN.LM22 and attach the supporting documents.

Applicants need to complete form JPN.LM12 and attach the supporting documents. Registration can be
made any NRD Office throughout Peninsular Malaysia.

2. Departmental Official Letter to apply for a search of the register (for application from Government
Department)
3. Any document or information that can be used as a basis to make a search of the birth register such

EXTRACT OF BIRTH CERTIFICATE IN HEDJAZ

APPLICATION PROCEDURES

Applicants need to complete form JPN.LMH3 and attach the supporting documents. Registration can be
made any NRD Office throughout Peninsular Malaysia.

Regional Registrar.

CORRECTING ERROR IN BIRTH CERTIFICATE

i. APPLICATION TO CORRECT CLERICAL ERROR

APPLICATION PROCEDURES

Applicants need to complete Forms X and JPN.LM21 and attach the supporting documents. The informant
must be the mother/father of the child. Applications may be made in Peninsular Malaysia and will be
forwarded to Sarawak for the approval of the Regional Registrar.
SUPPORTING DOCUMENTS

1. Personal identification documents of parents (if non-citizen)


2. Personal identification of guardian (if non-citizen) proven by Letter of Permission of Guardianship from
JKM
3. Letter of authorization from natural parents who give permission to guardian to make alteration
application
4. Personal identification document of applicant (if non-citizen) who exceeds 21 years old
5. Other documents proving errors

FEES

· RM 2.00 – Processing fee


· RM 5.00 - Approval
· RM 5.00 - New extract of birth certificate

ii. APPLICATION TO CORRECT ERROR OF FACT

Application to correct error of fact where the information other than the name of the child provided by the
informant during registration of the birth is incorrect or not available. The applicant needs to submit
supporting documents to prove the error of fact.
