The facility is able to handle a full census. At this time thay are only occupying 50% of teh beds due to low census. It is a poorly supplied and equipped
hospital for it size and services to the community. The director placed emphasis on financial needs and assistance. Oxygen, blood antigens, and financing

At this time this hospital in maintaining a census 133% of its capabilities. Staffing is an issue due to transportation. Pharmaceutical supply was a major

Assessed This facility is a large, medical-school affiliated teaching hospitals with organization and services which largely parallel a similar U.S. facility.
Equipment and medical supplies appear adequate for normal operations, and thehospital structure and administration are intact and functioning. The
greatest need for short-term assistance appears to be help with potable water, and generator fuel pending return of the city power grid. Indirectly, the
hospital would benefit from the reopening of community hospitals so that large number of patients who would normally be seen there will return to these
3 2 Baghdad hospitals and offload this facility to a normal workload.
This general teaching hospital is located in aged but very well-maintained buildings. The importance of this hospital stems not only from its role as a
community hospital and teaching hospital, but also from its role as one of a few dialysis centers, and one of only 2 or 3 hospitals performing renal
transplants, and one of only a few burn centers. There is a critical need for propane at the facility for food preparation. Gasoline is needed for the
ambulance and other facility vehicles. A report was turned into the 422 S-3 for an emergency supply of propane to be delivered. The facility is close to
functioning at its pre war level.. Supply with Kamadia has been working but critical shortages are still occurring. Several of these shortages have been
repaeated at different facilities. Electricity has not been an issue during the last two days but repair or replacement of the generators is essential due to
inconsistency.

sponsorship by a private organization who may be able to provide manpower, expertise, financing, and some equipment to provide for return of operations

This hospital is a typical community hospital, with well-motivates staff, many of whom live in the surrounding community. They are seeing a significant
increase in traumatic injuries which has expended their supply of external fixators and casting supplies. They are beginning to also see an increased
number of infectious disease cases, especially diarrhea. Short term needs are for these items, specific pharmaceuticals as mentioned, and lab reagents

Assessed The staff physicians warned us that the area in sensitive to US military forces and that they would not be welcome in the area. This hospital is a
moderate sized community hospital which seves a large residential population in a low SES area. It is old and the equipment is dated, but is providing the
bulwark of healthcare to its community. We recommended that the hospital director send a trusted snior representative to kimadia for supplies and
medications. The hospital also resquires a resupply of fuel to keep the fuel storage tank full. Some Civil Affairs activity must be initiated in the area to

American soldiers coming to scene thought there were political prisoners there and forceibly released patients from secure wards and are now on the
streets in Baghdad. As situation stablizied, 300 of the 1200 patients have returned. There is no power, water, security. Most critical need is security and
repair of walls. Several NGOs are attempting to assist and will redouble efforts once area is more secure. Need to track down criminally insane in

Assessed Facility damaged during war. Assessment on 16 April was that entire building would require renovation before patients could be seen. This
facility previously represented one of two such facilities in Iraq, able to offer comprehensive cardiac surgery capability for those with access to its care. The
staff have previously rebuilt this facility following servere damage during Operation Desert Storm. The current level of damage appears to exceed this
however, and the facility is probably not a candidate for renovation, but would need to be entirely rebuilt, and to a moderate degree, re-equipped. Currently,
steps should be taken to safeguard remaining equipment, especially cardiac bypass pumps, instruments, and catheters and supplies at a minimum, and to
3 3 2 Baghdad preserve the surgical staff and support staff as a functioning team if cardiac surgery capability is to be maintained.
Assessed TheCentral Blood Baghdad Blood Bank located in Medical City appears to have primarily aging equipment in need of replacement. The practice
of relying on individual patients transporting their own blood products to point of use seems incredible, as it offers no quality control in transport temperature
and ultimate use. Staff were questioned several times on this point and maintain that this is the primary method of dispensing blood products. Short term
2 Baghdad needs are for additional ELISA kits for HIV/Hepatitis screening, and multiple chaneel pipettes.
This facility is a busy, community level outpatient and inpatient treatment facility providing primary care and basic hospitalization capability. It is receiving
more patients now then pre-war, including adults. There are serious shortages of all classes of supplies and equipment. short term assistance should focus
on basic supplies and pharmaceuticals. The hospital needs to maintain the ability to refer seriously ill patients and provide for their safe transport to higher

Assessed This hospital is a small, private hospital and has survived relatively intact despite the war. The main diffuculty that is experienced involves the
lack of nighttime security and nedications. The hospital staff is experienced and well versed in the delivery of health care during these difficult times. The
hospital administrator speaks English well, is forthcoming with information, and appears to be very knowledgeable regarding the healthcare delivery system

Assessed This dental center serves the low-income area where it is located. Heavy looting and vandalism have rendered it inoperative at the moment.
Priority should be for electricity (replacing stolen generator batteries), security (including investingating the illegal arms dealer in teh area), repair of dental
equipment (probably requires technical assistance from a Siemens representative), staff salares, replacement of stolen air conditioners prior to the hot

potentially affecting operations include resupply of fuel and security for hospital staff and the hospital building. The next major issue is the resupply of
hospital medications and vaccinations. This hospital needs money to pay the hospital employees along with supplies of water. The director of the hospital

Assessed This hospital is a highly specialized tertiary care medical center with University affiliation. The senior leadership at the hospital received most of
their training in England. All speak king's English quite well. This hospital continued to the success of teh hospital. Short term issues include security,
salaries for the staff, and medical re-supply. Medim term issues include obtaining outdated equipment (see the attached list). In the long - term this hospital

security, return of city electrical power, and especially salaries for staff. The base salary for house officers in Iraq is $1 per month, an only senior staff have

Private surgical sub-speciality hospital now seeing patients on emergency basis only. The hospital normally functions by procuring payment fom its
patients. As it is currently performing emergency procedures only on an unpaid basis, the hospital is unable to replenish the supplies it hasused. In the
past, the hospital had paid for all supplies obtained from the MOH or by other means. We recommended that the hospital director send a trusted senior
representative to Kimadia for emergency supplies and medications necessary to continue its current purpose. The hospital also requires a resupply of

This is a large, specialized surgery center which is functioning at a very limited level curently. Normally a 200 bed facility with 17 floors. Backup power
allows for only 4 floors to be operational, and given the vertical layout of the hospital and specialized nature of each floor, effective operations are not now
possible. The prior regime parked a large fleet of new cars/trucks in the underground parking garage to protect them from war damage. The MEF
apparently parked the vehicles above ground, in front of teh hospital, and they have been target for looters, and "police" trying to accquire them. Priority
effort to remove and safeguard these vehicles should be taken. Regarding electrical power, this hospital and all Mecial City Complex facilities are powered
Baghdad by a single electrical substation. Priority should be given to restoring this substation as soon as possible.
This hospital is a large teaching hospital which serves as the referral center for the entire Iraqi health care system. Its capacity is currently limited by the
aboves issues. We recommended that the hospital director snd a trusted senior representative to Kimadia for supplies and medications. The hospital also

This facility provides comprehensive multi-specialty care to mostly wealthy patients. The patients were seen on a referral basis only. The main concerns of
the staff included power for sustained operation and salaries for workers. The staff also seemed very interested in treating patients, poor patients instead of
there usual referral of wealthy patients. This facility could serve well as a specialty referral center for the general public. The physicians weh staff this

Assessed Saint Rapheal is a private hospital that serves primarily as an outpatient, maternity, and general surgery facility. The hospital operates
essentially completely independently of the Ministry of Health. The hospital relies on provate charity and pay-as-you can receipts, and purchases second
hand medical equipment and obtains medications outside of the established State Company. Dedication of the staff and satisfaction of the patients served
is markedly above the norm for the region. Equipment is impeccably maintained even if old. This hospital would benefit perhaps more that most hospitals
from donatons of basid class VIII supplies. The hospital would also benefit greatly from an anesthesia machine and electro-surgical unit if these are
available from any abondoned former regime facilities. The hospital provides important basic services to poor patients regardless of religious affiliation as
1 1 3 Baghdad well as to wealthier patients who desire a greater degree of individual care and attention.
Assessed This specialty center is critical-operational due to lack of supplies and losses secondary to looting. Previously Samarri hospital was one of the
best infertility centers in Iraq. Now the hospital staff is unable to care for patients and perform complex and simple procedures. Security is major porblem
given the all female staff and the lack of armed security guards. Communications is another issue that will have to be aaddressed in the near future along
3 1 1 Baghdad with the purchase of many high tech devices. The hospital director is confident that this hospital will one day return to its former status.

equipment. Insulin is in short supply. During our visit it was brought to our attention by the ODA soldiers responsible for thsat 5 people have died recently
at the hospital due to lack of insulin. Linen is needed for the infection control. Replacement and repair of window air conditioning will be necessary due to
the heat of the upcoming summer. Oxygen delivery services are needed. The lab is lacking in reagents and kits and is needed of a system for testing

The building is sound, of modern functional design, and the staff numerous and well trained. Water, supplies, nursing support and equipment are woefully
inadequate. Basic surgical capability can be presumed, with probable capability for cardiothoracic, neurosurgery, ENT, orthopedic, and neurosurgical
capability. A significant weakness is likely to be in long-term stays, and post-surgical care due to probable limited supplies and inadequate hygiene and
infection control. While this hospital does not meet Western standards, it appears to be significantly better equiped and ataffed than a comparable facility in
the Former Republics of the U.S.S.R. by way of comparsion. The hospital appears to be transitioning from a no-fee to a partial fee-for-service system.
Providing adequate supplies and especially restoration of hospital-grade electrical power and water would greatly enhance the capabilities of this hospital.

Salah Aldeen General Hospital was previously a military hospital. As aresult of the war and the collapse of the Iraqi militaty it has been reopened as a
civilian hospital to serve the needs of the iraqi community of North Tikrit. It is now civilian staffed, though many are retired military physicians. The sub-staff
are all civilians workers, and were prior to the war. While the conflict was going on, most of the doctors fled to their hometowns outside of Tikrit and brought
some medical equipment with them to prevent looting. The building suffered cosmetic damage, primarily to windows and doors, during the conflict. The
electrical and water systems remained intact and the hospital structure was not significantly damaged. Of note, after the Iraqi Army fled, and the doctors
Tikrit returned they held a democratic election and voted out the prior Iraqi military hospital administrator, Dr. Sammer Hamon.
Hospital functioning at reduced capacity. Adiquitely staffed and supplied for now. Local generators and local well provide minimall sufficient electrical power
and running water. These are backup systems, however, and the staff is eager for the return of regular electric and water services. Many windows have

The Daquq hospital is a well running community hospital in relation to other facilities here in Iraq. The hospital does not appear to have been affected by
the war in any way. The hospital staff has been paid UTD with 99% of the staff showing up to work and appearing very happy, helpful, toward the American
forces. The relationship between the hospital and primarycare clinic is very solid with family members running both. There appears to be a mild shortage of
certain medication mentioned above but does not seem to disrupt operations. Some possible easy improvements for the hospital would be to get a heart
monitor, and life pack. This would greatly increase the emergency room department. Instruments are auto-claved and soaked in different knds of a-septic
solutons for sterlization.

The Hawaija General Hospital is intact and functioning well. While its capabilities are limited, they appear to have not been significantly degraded by the
war. The major concerns currently are shortages of some medications, and other supplies. They report that the major breakdown is from resupply and
getting equipmetn fixed is transportation. They say that to drive to Kirkuk and back to pick up supplies costs them 40,000 dinars for fuel. They attempt to
make the trip once a week, but when they get to Kirkuk they are told that even Kirkuk and Baghdad are short of supplies, so they receive a limited amount
of what was requested.

This is a small psychiatric hospital and is the national referral facility for short term stays of mentally ill persons and th teatment of addictions. The facility
also offers out-patient treatment for children with pschiatric problems. The hospital has only a small portion of beds filled due to rooms without air
conditioning and the inability of patients to travel to the hospital.
Jawlawla Hospital is the only hospital in this community of about 30,000 people.
Reported by the marines on 13 April. Hospital was fully operational.
The medical system in whole lacks a well-trained paramedic and nursing corps. The level of traininig of the doctors could not be properly assessed. The
senior doctors appear to be well trained with differing levels of experience in western Europe and in treating a multitude of combat related trauma patients
throughout a long period of hostilities. Due to restrictions on freedom of movementof the kurds, the younger doctors will obviously have less training and
experience, although this might be made up in mentoring from the senior doctors. The doctors spoken to were professoinal and portrayed a realistic but
"can do" attitude for maximum patient care with limited supplies and equipment.
