I, Sri.Panda Mani Prabha submit that my father Sri. Panda Ganapathi Rao while
working as Bunglow Watcher, Kasibugga Range of Srikakulam Division was expired on
19.04.2018 which is natural death due to ill health contain Death Certificate issued by
RIMS, Srikakulam vide Certificate dated 26.04.2018 (Xerox copy is Appended)
I submit that the Tahsildar, Srikakulam has also issued Family Member Certificate
contain the following are the family members of the deceased individual which contain
D.Dis.No. dated (Xerox copy is Appended)
01) Smt. Panda Santhi Kumari Wife & 1st Legal heir of the deceased
02) Sri. Panda Mani Prabha Son & 2nd Legal heir of the deceased
03) Sri. Jagannadham Purnima Marriage Daughter & 3rd Legal heir of the
deceased.
I submit that Government through their orders under references cited with time to
time amendment and clarifications given it was enlighted to provide employment to
one of the family members of the deceased as per the norms and eligibility subject to
imposing certain conditions/guidelines to fulfill them.
I submit that due to the death of my father are entire families is in harmness and it
is necessary to provide suitable job to me to maintain the family for survival and
livelihood. In compliance to the instructions of the Government issued through various
G.Os. Subjected to production of certain documents I opt seeking appointment as
Junior Assistant in one of the offices of the department being the Collector and District
Magistrate, Srikakulam is the Nodal Agency of the District as there are no vacancies for
me in Srikakulam Forest Division.
 I submit hereunder the following Xerox copies of documents for kind perusal as
required to consider my representation for appointment as Junior Assistant under
compassionate grounds to seek livelihood of our entire family for survival.
01) Death Certificate issued by the RIMS, Srikakulam.
02) Family Member Certificate issued by the Tahsildar, Srikakulam.
03) Educational Qualifications contain Date of Birth of SSC Certificate.
04) Educational Qualifications Certificates in having myself passed B.A. Degree.
05) Notary Affidavit contain No Objection from my mother married sister in the
event of my appointment to suitable post.
06) Caste Certificate contain “Vaddi” (O.B.C) Caste issued by the Revenue
Authorities.
07) Self Declaration Certificate for No Earning Members in my entire family.
I finally submit that considering my present application basing on the Self Declaration
Certificate issued by me and Consent letter obtained from my mother (L.R-1) and from
my sister (L.R-3) I may kindly be appointed as Junior Assistant in one of the offices in
Srikakulam District by virtue of my qualifications, age and eligibility to support my mother
depending on me as my above elder sister was married already and maintaining
separately.
I pray early action to issue orders and directions as deem fit proper on its merits to
help our entire family providing job under compassionate grounds of appointment to
extend the livelihood for survival at the earliest.

Kasibugga Range of Srikakulam Division do hereby offer my self declaration
declaring that there are no Earning Members in our entire family and the only
Widowed mother by name Smt. Panda Santhi Kumari is depending on me and
there are no other source of assets left over by my father at the time of his death
and this declaration is deposed at my own accord as per privilege given by the
Government and in the event of my declaration found false at any time I am
liable for any action proposed by the Administrative authority.
