Training Basics.

1. Orders

– enter the verbal order in the computer, print, sign


and date it and give to case manager, (or place in
orders bin and case manager will pick it up) who
will then take care of sending it to the nurse or
nursing agency.

- place signed hard copy of order in “filing” bin to


go to the chart.

- can also be used as a communication tool for;


your pharmacy technician to notify them of
changes in doses that might impact the delivery
schedule, a pharmacist that you are covering for
or when on call.

2. Filing

– place in filing bin any thing that needs to be in


patient’s chart, e.g. signed copy of physician
orders, admission packets, etc.

3. Faxes

- fax cover sheet with fax # to our team (John and


Mark’s) 942-3218

4. Labs

- schedule in computer when on call and share with


Kim Dillion. Otherwise Kim will schedule labs in
the computer for us.

- Pharmacist faxes labs

- Involve your case manager when problems occurs


especially when dealing with outside agencies.
Have your case manager call and find out whether
the labs were drawn or what happened.

5. C-II Rxs

- Pharmacist is to fax the “prescription info” to the


physician (using our Amerimed fax cover sheet).
Place a copy of the drug label on the lower right
hand corner of the sheet.

6. Checking on therapy DC

- Start each morning by printing out your patient to


contact deliver list. Meet with your case
manager to decide who is calling MD to
check on DC.

- The pharmacist will any changes in dose, change


to cath care, etc to patient.

- when do we enter as new therapy for cath care?

- If the initial AOB does not address cathcare, then a
new AOB should be obtained and new scripts
generated under a cath care therapy.
- For any drug change or change from IV to cath
care, check with Doug or Lisa to make sure we
have the correct AOB. Likewise with Enteral
formula changes, check on the AOB or when the
rate changes form say 4 to 7 cans a day.

7. Outside Agencies

- med profiles: these are sent from outside agencies


to the case manager who notifies the pharmacist.

9. New Order Checklist

– copy of physician orders

– 2 copies of drug monograph, 1 for patient to keep


and the other for patient to sign and us then file in
the patient’s chart.

– physician script

- programmed pump if needed. Remember to send


new pump if infusion volume changes e.g. with
TPNs. If sending a new pump, print out a pick up
ticket for the old pump.

11. Hospice Patients

- Pharmacist take all hospice referrals, then


transcribe to hospice referral sheet.

- request signed physician order, patient


demographics, allergies, ht and weight.

13. Cath Care Patients


– send a minimum of 2 weeks supply
– change theray type to Cath (not Flush)
– send gloves only if Amerimed nursing
– do not forget max connector and 15 inch ext set
– Power PICC: Kettering treats this as a regular
PICC, i.e. send saline and heparin. Reid ANC and
Community treats their power PICC as a groshung,
i.e. no heparin.

14. Nursing Home Patients
- send baxter pump
- send meds in bags (no IV push or eclipse)
- send drug, tubing (1 per day), heparin, saline
- if PIV, send IV start kits and angiocaths

15. CADD Pump Intermittent orde entry


- container is sodium chloride with the correct bag
size
- dose per day is the # of doses in the bag e.g 18
MU/500 ml bag at 3 MU q4h is 6 doses/day
- doses dispensed is doses/day X # of days
dispensed e.g. if sendind 7 days, doses dispensed
= 42
- container dispensed = # of bags dispensed
