1. INTRODUCTION

1.1. Description
This Code of Practice (CoP) is written to set policy and the minimum standards required for data
entry into OpenWells and PROFILE (the Group replacement for DIMS). As with DIMS,
OpenWells is a Landmark Graphics product and it is part of an integrated suite of applications
for Well Engineering which are supported by the common Engineer’s Data Model (EDM)
database platform.
This CoP will explain the strategy, the logic behind the hierarchy attributes, the operations and
NPT coding systems.

1.2. Applicability
This CoP applies to all employees within Wells. This includes all Well Engineering and Well
Services employees.

1.3. Language
The words shall, must and will indicate mandatory requirements and will require an approved
variance for deviation.
The words should and may indicate recommendations

1.4. Regulatory Requirements

Each Region has it’s own Regulatory Requirements.
OpenWells allows for the capture of various operations milestones in the EDM Database.
Management of Regulatory Requirements will be handled locally.

1.5. Transition
Existing DIMS Data will be migrated to OpenWells as it is implemented.
EP 2005-0000 -2- Restricted

2. STRATEGY AND PRINCIPLES

2.1. Strategy
• To create a Group common standard for Well Operations operations reporting.
• To have common well, wellbore and event attributes to allow classification that
will ensure ‘like’ well operations performance can be compared on a regional and
global basis.
• To create a reporting structure that reduces the chances of error in data entry and
provides data that can be used with confidence for performance monitoring and
to predict future projects times and costs.
• To facilitate Shell Group internal reporting and benchmarking of well operations
performance.
• To align well operation phases with the desired detail level required to produce
consistent ‘fit-for-purpose’ well activity durations and costs.
• To create an activity reporting structure that enables the DTL work process.
• To enhance Well Services reporting.
• To facilitate capture of all necessary mechanical well data in order to be able to
produce a Well Status Diagram that satisfies the requirements of future well
entries and all Safety Critical Integrity management requirements throughout the
well life-cycle.

2.2. Aim
• The aim of this CoP is to describe policy and standards for the application and
use of OpenWells, Profile and Data Analyser applications within the Shell Wells
organisation.
• All applicable Shell and Landmark training materials shall support this CoP.
• The CoP for operational reporting is a set of Shell Group agreed minimum
standards that operational reporting shall adhere to.
• A description of how the EDM data hierarchy is structured is included.
• Non Productive Time (NPT) and Lateral Learning principles are explained.
• A description of typical roles and responsibilities for a Region or Company are
laid out in matrix format (Appendix 1).

2.3. Classification and Principles

With the adoption of OpenWells comes a new coding structure for recording
Well/Wellbore/Event header and operational performance data. The coding system has been
created after consultation between the regions that make up the Shell group. The adoption of a
group common coding system will result in benefits to the benchmarking process. There will be a
greater emphasis on internal benchmarking.
EP 2005-0000 -3- Restricted

Considerable enhancements have been made to the system in order to help with consistent
reporting. These are:
(1) An improved classification of wells and operations executed on the well.
(2) A simplification of the phase coding system to mark milestones in a wellwork
program, which will ensure no bouncing in and out of phases.
(3) The introduction of a primarily two level recording system with a third level
assigned exclusively to Well Servicing, including wireline and well intervention
operations.
(4) The capture of all well services operations

(5) A common NPT and Non Conformance reporting and closeout process.
(6) The potential for the automatic extraction of data for well planning, business
improvement (DTL Analysis) and NPT and Non-Conformance reduction. This
also recognizes that there will be a heavy reliance on Phase, Operations and NPT
data to be entered accurately.
(7) A Lateral Learning functionality with the capability to record learnings at all stages
of the Well/Life Cycle.
NOTE: Persons responsible for the recording of operational data in the OpenWells
database must be familiar with the new Phase, Operations and NPT categorisations

2.4. Benchmarking and DTL

Shell has also subscribed to global benchmarking services such as Rushmore and Dodson. This
has proved to be a time consuming exercise. The implementation of EDM offers the prospect in
the future of doing our own benchmarking globally, and simply taking benefit from the standard
application.
The adoption of tools such as Spotfire along with the capability to hold all Regions’ well
engineering data in a common format makes internal benchmarking entirely feasible.
Traditionally the prime purpose of entering data into databases has not been to analyse
performance but to produce the daily drilling report. This emphasis is now changing with the
global introduction of probabilistic well cost modelling and the consequential reliance on accurate
phase durations and NPT event recording.
Similarly the success of DTL relies on being able to quantify what is capable of being achieved on
an activity basis and providing a mechanism where challenges can be made on rig performance,
the business and thus the bottom line.

2.5. OpenWells
OpenWells is Landmark's latest Drilling and Completions Optimisation reporting application. It
is a fully comprehensive well operations reporting, communications and data analysis system
integrated into the Engineer's Desktop.
EP 2005-0000 -4- Restricted

This application is used to capture well operations information for operators or service
companies that can be used for performance analysis or analysed using engineering applications
on the Engineer's Data Model (EDM) platform. OpenWells shares a common database with
CasingSeat, StressCheck, COMPASS, and WELLPLAN.
OpenWells provides drilling, completions and well servicing information within a single
application, supports multiple systems of measurement and can be easily configured to support
multiple languages. Whether exploring, developing, or managing your asset portfolio, you can
quickly procure a complete operations history for any well through OpenWells.
NOTE: All NPT, Lateral Learnings and Root Cause Failure Analysis (RCFA)
reporting MUST be done in OpenWells.

2.6. Engineers Data Model (EDM)

An integration platform for drilling, well services and potentially production and
economics data.
The Engineer’s Data Model (EDM) provides the foundation for the next generation of well
planning and operations reporting systems. It offers seamless integration between Drilling and
Well Services applications through a complete well construction solution. EDM is designed to
support engineering workflows with COMPASS, WELLPLAN, CasingSeat and StressCheck as
well as OpenWells through consistent Data Management, Navigation, Security, Unit Handling,
Reference Datum Levels, and Multi-Application Concurrency. EDM provides a single platform
for detailed operations and engineering workflows from prototype, to plan and actual phases of
drilling or servicing a well.
The EDM platform enables engineers to realise valuable productivity gains during the well
planning process through efficient naturally integrated workflows. It offers integration of
applications, ensuring one data entry point for common information, and guarantees data quality
while supporting well design and operations. EDM provides the foundation for making real-time
engineering decisions. A current operation can be analysed easily with any of the engineering
tools, based on the latest operational parameters from the daily operations report. Additionally,
EDM provides an integration platform for third-party applications.
NOTE: Well planning data MUST be entered into EDM, and this therefore is a
critical success factor.

2.7. Tight Groups

Tight Groups are used to restrict access to the Site and Well levels of the Well Explorer
hierarchy. Each Site and Well is associated to a particular Tight Group. Sites and Wells to which
the User or their Group does not have Tight Group access are not displayed.
This security measure prevents the user from accessing this data. For a Restricted Site all Wells,
Wellbores, Events, and Reports within that Site are hidden. For restricted Wells, all Wellbores,
Events, and Reports are hidden. The System Administrator using the EDM Administration
Utility can manage Tight Group creation and membership.
EP 2005-0000 -5- Restricted

3. CODING STRUCTURE

3.1. EDM Tree Structure/Data Hierarchy

When working in the EDM applications (OpenWells, Profile etc.) an identical approach is utilised
across the applications to locate the well that is to be worked on. Navigating through a data
hierarchy laid out in a tree structure does this.
The elements that make up the data hierarchy are as follows:

3.1.1. Database
This is the database name that you will select when logging on to OpenWells.
A set of regional databases has been created, residing in Amsterdam, or locally in the
Region as follows:-

EPE Amsterdam

EPW Amsterdam

EPM Amsterdam

EPT Amsterdam

EPA Miri

BSP Seria

PDO Muscat (Planned)

EPG Amsterdam

SPDC Port Harcourt (Planend)

SEIC Yuznho (Planned)

Each Regional database is prefixed EPE_, EPW_, EPM_, EPG_ and EPA_ as
appropriate. The second part of the database name will then be EDM_P1 where P
(indicates the operational or Production database).

3.1.2. Company
• A Company may have several Projects
• The term company may represent an operating group within an exploration and
production company
EP 2005-0000 -6- Restricted

• A company has common directional survey data management practices and

policies
Each region may find it necessary to establish a number of companies; such as:
EPE_Aberdeen
EPE_Assen
EPW_GOM
EPW_RMP
EPG_SPDC

3.1.3. Project
• A field, asset or group of sites
• All Sites within a Project are aligned to either Grid North or True North
• Project is the data level directly above the Site level
• Each project must have a unique name within the Company
• One system datum is used for each project (e.g. mean sea level that is used to
define 0 TVD for the Project)
• Depths can be referenced to the Project level system datum or to additional rig
depth reference elevations specified at the Well level
Examples of projects from EPE would be Brent, Cormorant etc. EPW project examples are
Auger, Brutus etc.

3.1.4. Site
• A collection of one or more Wells referenced from a local coordinate system
centred on the Site location
• May be an offshore platform, single land well, single subsea
Exploration/Appraisal well, or cluster of subsea wells e.g. on a template
• Data level directly above the Well level
• Must have a unique name within the Project
Examples of EPE sites would be Brent Alpha, North Cormorant etc.

3.1.5. Well
• A Well is a surface or sub sea wellhead location on which Rig or work units
conduct operations down the Well and may have one or more Wellbores
• Data level directly above the Wellbore and Event levels
• Must have a unique name within the Site
• Wellbores may include the original Wellbore with one or more Sidetracks tied on
at different kick-off depths

3.1.6. Completion
• A system of isolated wellbore openings, wellbore conduits, and equipment, which
provides a means to produce or inject fluid (gas or liquid) from or into producing
zones.
EP 2005-0000 -7- Restricted

• Single Completion may provide the conduit for fluid flowing via one or more
Effective Wellbore Opening Intervals:
• Effective Wellbore Opening Intervals servicing one Completion are contained in
the same Wellbore

3.1.7. Wellbore
• Describes path of a Well from a top depth to a bottom depth
• May be a planned (or unplanned) sidetrack or lateral in a multi-lateral completion
• Data level at the bottom (same level as Event) of the data hierarchy for drilling
located beneath the Well
• Must have a unique name within the Well
• Original Hole is entered as a Wellbore and would start from surface.
• Wellbores may start some distance along an existing Wellbore - a "Sidetrack" i.e.
Wellbore 2 etc.
A well commences with the original wellbore, which is referred to as Wellbore 01. Subsequent
wellbores are added as the original wellbore is sidetracked i.e. wellbore 02 etc.

3.1.8. Event
• A time slice or period marking a phase of major operations occurring on a Well,
Refer to Section 4.1 for details.
• Data level at the bottom of the data hierarchy for operations in the Well or at the
Well Site
• Create a new Event when the focus of work activity on the Well changes
• Every Well can contain multiple Events
• No concurrent Events or overlapping Events
• A single event can only relate to one wellbore
Examples of events for EP operations are Drilling Only, Completion Only, and Workover etc.

3.1.9. Report
A Report is a subset of an Event where operations and mechanical well details are recorded.
As can be seen the data hierarchy is such that a large development can be categorised in order to
keep data separate. This may not be suitable for single well exploration ventures. In such cases it
is quiet all right to have duplication of naming within the levels.
The following screenshot from te pplication makes the hierarchy much clearer:-

4. ACTIVITY DESCRIPTION
There is now a Group wide agreed coding that has the following features.

4.1. Well Event Type

The Event Type is a description of the primary purpose of a visit to a Well for an activity on a
Wellbore by a single Work Unit. All Events must be associated with activity on a well. Reports
of Operations within an Event will be associated with a Wellbore. Events can take place on a
well throughout its lifecycle. The following Event Types are defined:

Event Type Description

Preparatory Work Describes the event where a work unit visits a well/wellbore for
(PRW) the purposes of preparing that well/wellbore for further activity.
For instance a separate work unit might be utilised to set
conductors for a development project.

Rig Move/Rig Down Describes the event where the primary purpose is to move the rig
(RIG) on or off location. This activity may often be associated with a
“Utility Well” where such activity is booked as the work is not
specific to an individual well or wellbore. Note: Where a rig
move is part of a wellwork activity on an individual well, then the
rig move would be part of another Event Type.

Drill Only The original drilling of a well/wellbore. This would be the event
(DRO) type for most exploration and appraisal wells for instance Note:
testing activities on an exploration/appraisal well would also come
within this Drilling event. Since a new wellbore in an existing well
is a sidetrack, this event type also covers sidetrack drilling.
Drilling and Used in the past for a lot of development wells (particularly
Completion offshore) where it was the intention to drill and (permanently)
(DRC) complete the well/wellbore at one visit. Given current work
practices, it is strongly suggested that this Event type IS NOT
USED. It will continue to exist to satisfy the requirement of
historical data. Separate Drill Only and Completion events
should be chosen for such an activity in the future, and Output
reports will be configured to allow the viewing of multiple Events
in a single report e.g. Drilling and Completion.
Completion Only Describes a visit to an existing wellbore for the purposes of
(COM) installing a completion including associated wellhead/tree
components, and potentially perforating. This would be the case
both for rig installation and where for instance a Production Hoist
is used to install the completion after a Drilling Unit has left the
location.
EP 2005-0000 - 14 - Restricted

Event Type Description

Well Information An event within a wellbore where the primary purpose is to collect
(WIF) information in relation to the well, formations or production
performance of the well; e.g. Production log or Production test
Well Intervention An event where the primary purpose of the activity is to modify
(WIN) the well/wellbore in some way. This could for instance be by
adding perforations, or the setting of a temporary or permanent
plug in the well. Note: The work will not involve change-out of
tubing or xmas tree.
Well Maintenance Work which entails performing activities on a well/wellbore to
(WMA) maintain the permanently installed equipment. This could for
instance be the maintenance or replacement of wellhead valves
or subsurface safety valves.
Workover An event that involves the replacement of the original production
(WO) tubing in a development well. Note: Any work within a
well/wellbore, which does not require pulling and re-running of the
tubing, should be treated as a well intervention.
Abandonment Describes the event associated with the final abandonment of a
(ABA) wellbore and/or well. For instance a visit to a suspended
exploration well, which is being finally plugged and abandoned.
Note: Abandonment of a wellbore prior to sidetracking will be a
separate event in the original wellbore.
Utility/Suspended Again an Event often associated with a “Utility Well”. Should be
Operations used for instance for rig-upgrade or rig maintenance activities
(UTS) which are not associated with any specific well or wellbore.

Clarification Notes
(1) Activity on multiple wellbores in a well (e.g. a multi-lateral well) will each form a
single Event, and the reports associated with these activities will be associated
with the individual multi-lateral legs (wellbores). The first activity within the
Event would cover drilling of the parent wellbore, together with all activities up to
the point where new formation is drilled in a second wellbore (i.e. on completion
of POOH the window milling assembly). Subsequent operations (i.e. picking up
and running the BHA for drilling) would be recorded against the new wellbore
and new event.

(2) Sidetracking of an existing well will consist of 2 Events. The first covers
abandonment operations on the existing wellbore and preparation for
sidetracking, including e.g. whipstock setting and casing milling up to the point
where a BHA is to be picked up, run and new formation drilled. The BHA
operations would constitute the first operations on the new wellbore and would
therefore be a separate drill only Event.

(3) Unforeseen activities that interfere with the progression of work (e.g. security and
weather related delays for instance) can be captured within the Event that the
activity occurs on, or alternatively for extended delays, may be recorded under a
EP 2005-0000 - 15 - Restricted

separate Event against the well or Utility Well for that installation. These
activities will mostly likely constitute NPT. The choice of whether to capture
against the well or dummy well, will depend on local budget handling of such
delays.

(4) Batch conductor setting (drilled and set), drilling or completion must be handled
by separate Events against each well.

(5) Conductor installation operations would typically be handled as a Preparatory

Work Event, as long as no drilling takes place. If pre-drilling is required to
achieve adequate conductor setting depth, then this should be handled within a
Drilling Event as per (4).

(6) Typically on for instance a Production Platform it will be necessary to create a

“Dummy Well” or “Utility Well” to assign Events to, for events which are not
directly associated with any real well or wellbore. This could be for instance to
log rig mobilisation activities which are associated with a multi-well programme,
or extended rig modification activities for example.

(7) On a production installation, it may be possible to carry out certain preparatory

operations on a well e.g. prior to sidetracking, without the use of the drilling rig.
Such activities should be recorded in an appropriate event separate from
subsequent rig intervention. In this case use a Preparatory Work Event (e.g. for
wellhead bolt lubricating and loosening) or Well Intervention Event (e.g. for
wireline plug setting operations) as appropriate.

(8) A clear differentiation has been made in these Event Types between Workover
(tubing removal) and Well Intervention (work within the existing tubing).

4.2. Phase and Operations Definitions

A predominantly two-tiered system will be employed to categorize data. The first level is
phase with the second being operation. There is a third level to provide further detail on
specific well services operations such as wireline and CT work. This depth of analysis has not
been available up to now.

The principal changes to the phasing compared to DIMS are as follows:

(1) Hole sizes have been grouped to reduce the number of codes. (It was not possible
to reduce to one drill phase and use hole size as the differentiator).

(2) Core, Evaluate and Secure Hole have been added to ensure well planning and
performance measurement match.

(3) A Pilot Hole Operation has been introduced to cater for both surface operations
and reservoir tagging operations.

(4) Start and end points are clearly defined. These signify milestones in the well
activity and do not correlate exactly with previous definitions but will reduce error
in data entry.

(5) There is a much reduced chance of reverting to a previous phase once the next
phase has commenced.

(6) Phase definitions for major workovers have been simplified.

Refer to Appendix 2 for examples of typical Phases for some common event types

4.4. Operations
The “Reason” field has now been removed and two levels of operation reporting are now
available. Simplified examples are shown below.
Drilling Operations will mainly work on Phase and Operation.

Phase Operation 1 Operation 2 Description

D12 DRT M/U BHA & HWDP and RIH
D12 REM Ream Last 200ft to bottom
D12 DRR Drill from 4500-4550ft

There are Well Services codes in Operations (Level 1) and additional, detailed activity codes
exclusive to Well Services in Operations (Level 2) to enable performance measurement.
Refer to Appendix 3 for a full listing of Codes and their Definition.
EP 2005-0000 - 19 - Restricted

Well Service performance is predominantly being measured in two levels, as there are far fewer
phases associated with Well Services Operations. A short example showing how this works is
shown below.

Phase Operation 1 Operation 2 Description

IC LSR Land Hanger & retrieve landing string
IC SLO SAF Held toolbox talk with wireline crew
IC SLO RUD Rig Up slick line sheave etc
IC SLO PLI RIH, Install plug &POOH
IC PRE Pressure test tubing to 3000 psi

There are over 100 Operations codes in total.

These codes represent activities that may be normal or recovery from event operations. NPT will
be identified in a column in the daily activity entry screen.
The following Operations (Level 1) codes are the only codes that shall cascade to Operations
(Level 2) Well Services.

CODE OPERATION (Level 1)

BLO Braided Line Operations

CTO Coiled Tubing Operations

ELO Electric Line Operations

SLO Slick Line Operations

HWO Hydraulic Workover Operations

Appendix 4 details the allowable Operation Codes per phase. The system is configured such
that only these reduced sets of operations codes are available for selection when operating in
the various phases. The intention is to make it easier to select appropriate operations codes. If
during the course of operations additional operations codes are found necessary then this must
be requested via your local EDM Focal Point.

5. NPT REPORTING

5.1. NPT Definition

Non Productive Time and Trouble Time have been used differently across the Group.
Trouble Time incorporated activities that were still making progress, albeit less than the desired
rate. This will no longer be reported, as it is too judgemental and was being inconsistently
applied. It was also felt that this could lead to attribution of blame, which is more difficult to do
for legal reasons in some countries.
The concept of Known Invisible Lost Time has been dropped. Genuine NPT was being entered
into this category in order for rig teams to meet NPT targets. The Invisible Lost Time
measurement from the derivation of DTL, Gross Time and NPT remains the cornerstone of our
business:
ILT = Actual Time - Technical Limit Time - NPT
Non Productive Time is the time taken to perform all those activities that DO NOT
continue to progress well operations towards their objective. Non Productive Time
will also include any subsequent time taken to reach the same stage in the well work
programme prior to the NPT event from occurring i.e. reach the same depth before
the stuck pipe and back-off etc.
Extra bit runs, or casing cutting runs is not considered NPT. This will show as ineffective
operations and will show up in the Net Time analysis of an operation and this process will not
affect the estimation of well activity durations.
The second inconsistent area of NPT application has been addressed. If a well has been
suspended because of an NPT event then all activities undertaken to reach the point of the well
programme where the NPT event occurred shall be considered as NPT. If a well has been
handed over and a failure has occurred requiring well intervention then no NPT will be attributed
to these activities apart from those caused by new NPT events occurring due to the remedial
work.

5.2. NPT Reporting Standards

The minimum standard for NPT reporting is as follows. NPT shall be reported using one list for
NPT Benchmarking and a second list to allow meaningful ‘drilldown’ from this list.

5.3. How to Get Started After a Trouble Event

The Rig Superintendent and the Engineering Team Leader are responsible for the evaluation of
each event, to define the level of response using the attached “Trouble Event Severity Matrix”’
below), to develop a Terms of Reference (TOR) to address the response and to suggest an
appropriate team to handle the work.
The TOR should be completed within two weeks of the event occurring but sooner if possible
Note that this applies to the TOR only. The timeframe for completion of the full RCFA is
dictated by the severity of the event and the urgency of taking remedial action, to be determined
by Rig Superintendent, Engineering Team Leader and, if appropriate, Level 3 and 4 managers on
the basis of the ranking of the event in the Trouble Event Severity Matrix. It is, however, the
expectation that most RCFA’s are completed and closed out by the time the post-well AAR takes
place.
The Terms of Reference, at a minimum, should define the objectives and deliverables of
the investigation and provide appropriate background information on why this action is
being taken; typically, this would be a description of the event, a description of the
actual and potential consequences and some discussion on frequency of occurrence.
Also the TOR would include the names of the responsible party, the team leader and
the team members and due date for the action to be completed.

Response Matrix for Well Trouble Events below provides guidance for the appropriate
team makeup.
The Rig Superintendent and Engineering Team Leader should determine the severity of the event
considering both actual / potential consequences of the event or inability to achieve defined well
objectives and the frequency in which the event has occurred in industry, in EP Wells and within
a specific location. As an example, location could be Gulf of Mexico operations or a specific field
/ rig.
For consequence, use the highest impact either actual or potential. For frequency ask how often a
similar event has occurred. It is important to consider the potential of events so that escalating
factors can be understood and the appropriate level of response identified and implemented.
For events with a “severity level” of A, B or C, refer to ‘Severity Matrix for Well Trouble’ below.
The Rig Superintendent and Engineering Team Leader have authority to implement immediate
action.
For events with “severity level” D or E the appropriate Level 4 or Level 3 manager
input to and concurrence with the TOR is required before initiating the response.

5.3.1. How to Conduct the Investigation

In describing the event, what, where, and when the event took place should be clearly stated. An
estimate should be given of the trouble or cost associated with not achieving the well objective
with frequency and potential of the incident.
EP 2005-0000 - 24 - Restricted

Depending on the nature of the problem and practical limitations, the RCFA can be done in a
formal session or the RCFA Team Leader can collect information and base the analysis on the
obtained information.
As the threshold for low severity events (i.e. certain A or B events) is low, such that there may be
many of these events to report, the decision may be made by the rig superintendent and the
engineering team leader not to follow up on these events. Good judgment on the part of the
RCFA accountable party (see Response Matrix below) is required here. Particular attention needs
to be given to the re-occurrence and frequency of a particular A or B event. If the frequency /
chance of re-occurrence is high and the cumulative impact of multiple low-severity events is
significant, it is prudent to follow through with a thorough investigation.
Recommended tools for events with severity level C, D or E (refer to Severity Matrix for Well
Trouble and Response Matrix for Well Trouble Events below) are TapRoot and Tripod Beta but
equivalent tools can be used. Both TapRoot and Tripod are also used in the HSE incident
management process. TapRoot should be used in conjunction with EquiFactor in the case of
equipment failures. HSE staff may be available to help out with utilising TapRoot and Tripod
Beta tools.
In conducting the investigation, attention should be given both to the action(s) that led to the
event and the condition(s) that made it possible for the event to happen. Ask yourself 5 times the
question “why”, looking for the underlying reasons. If an error was made, look for what led to it.
After the root-cause has been established, the investigation team should recommend remedial
actions as appropriate. Consider cost/benefit for recommended actions and apply ALARP
principles. The recommendations will be evaluated by a Regional Technical Expert (RTE,
formerly named SME = Subject Matter Expert) before being implemented. For events with
severity level C, D or E (refer to Severity Matrix for Well Trouble and Response Matrix for Well
Trouble Events below) it is recommended to include the RTE as a regular member of the RCFA
team.
As indicated above, events that are considered low severity (i.e. certain A or B events) may not
warrant a full RTE review. Again, good judgment by the RCFA accountable party is required
here. In case there is no full review, the RCFA accountable party and RTE may have a brief
conversation (e.g. by e-mail or phone) followed by efficient closeout of the RCFA report (see
Documentation and Communication Process below).

5.3.2. Documentation and Communication process

The Response Matrix describes the levels of documentation required. At a minimum, each event
along with appropriate recommendations and follow-up should be summarised in ‘OpenWells’
using the NPT Report. The process for initiating the report, tracking progress, implementing
recommendations and closeout are outlined in the five-step process below.
EP 2005-0000 - 25 - Restricted

STEP 1 – The RCFA responsible party (or delegate; note that delegation applies to reporting
only; the RCFA responsible party remains the key responsible/accountable party during all the
steps of the process) initiates the NPT report in OpenWells. General, NPT and Equipment
Failures Sections are completed as follows:
• RCFA Reporter (i.e. the person filling out the RCFA report)
• Classification within response matrix (Refer to Figure 5.2 below) determining this
classification
• Time lost in hours
• Trouble / learning code, according to OpenWells NPT codes
• Learning and Completion/Review date
• Cost impact (in USD)
• Summary and Incident detail, including the TOR
• Classification of the NPT event includes:
o Source of learning (e.g. wellsite)
o Equipment type
o HDI main & sub categories for characterizing event / incident
o Potential “worst case scenario” severity matrix classification
o Owner/Manufacturer, hole size and depth
See OpenWells handbook for further details
STEP 2 – Detailed root cause analysis of the failure is carried out by the team assembled by the
RCFA responsible party (or delegate). The appropriate regional technical expert (RTE) is
identified. It is recommended to include the RTE as a member of the RCFA team for more
severe events (C, D & E). NOTE: in the case where the rig superintendent and engineering team
leader decide not to follow-up on low-severity events (i.e. A & B events), they should contact the
RTE and arrange for an efficient close-out of the NPT report. Note that initiation and
completion of an NPT report is still required even when events are not analyzed using RCFA.

STEP 3 – The underlying cause and recommendation is communicated by the RCFA

responsible party (or delegate) in the “Root Cause” Section C out/Comments close input screen
of the NPT report. Specific recommendations and actions are indicated in the “Preventative
Action” Section input screen. The report is formally offered to the previously identified RTE for
review by filling out the header information in the close out section.
STEP 4 – The RTE is notified of the completed NPT report by the NPT coordinator, who takes
care of weekly accumulation, tracking and notification on all outstanding NPT reports. The RTE
now takes the following actions:
• Review of the NPT report and recommendations / proposed actions therein;
• If necessary, the RTE improves on the recommendations / proposed actions by
modifying the NPT report after checking with the RCFA responsible party. Any
remaining disagreements are escalated to the secondary RTE and, if necessary, to the
Level 4 or 3 management for arbitration;
EP 2005-0000 - 26 - Restricted

• If warranted, the RTE creates or (if already available) updates a Practices Worth
Replicating (PWR, formerly known as best practices) document. These documents
include best practices that are shared regionally and globally. The PWR process is live
and full details can be found at: http://sww-pwr.shell.com/pwr/index.jsp

STEP 5 – After checking with the RCFA responsible party, the RTE completes the relevant
section of the NPT report. Either the RCFA responsible party or the RTE closes out the report
by checking the “completed” flag on the Close out Comments section report. This will remove
the NPT report from the list of active / outstanding report tracked by the NPT coordinator.
It is expected that actions and recommendations made in the NPT report are addressed /
implemented in a timely fashion, and are discussed in Drilling & Completions Network meetings.
NOTE: for more significant events (i.e. C, D & E events, see Appendix 1) Taproot and
Tripod Beta reports may be required (Refer to Figure below). These reports are to be
attached to the NPT report in OpenWells.

In the NPT report, the event and its position in the severity matrix above is indicated in the
“General” input screen. In the box “Response Matrix”, the event is coded as (consequence /
frequency). For instance, a consequence of 1-5% of AFE (line 2) and a frequency of “happens
several times per year in EP Wells” (column 40) is coded as “2.d”; a consequence of 5-10%
of AFE (line 3) and a frequency of “never heard of” (column 10) is coded as “3. a”, etc.

6. DATA MIGRATION
The move from a three tiered to a two tiered approach will have an impact on the data being
migrated and the ability to analyse historical performance. The ability to analyse will be enhanced
but the quantity of the historical data shall be reduced.
The new coding system is essentially building for the future, particularly at the two operations
levels.
Phase durations will be marginally affected but new phases should be within 10% accuracy
compared with manual conversion. The error will arise due to slight changes in phase definitions
and automated transfers where NEV codes have been used in EPIDORIS databases. A script
will have to be written to work around this issue.
The migration of the operations data shall be predominantly from ‘Operation’ to ‘Operation
Level 1’. ‘Reason’ instead of ‘Operation’ have been used for critical ‘Reasons’ such as “Well
Control”, BOP Testing” etc.
The Global NPT codes should be capable of being created without any problems from the
existing NPT data and the Global detailed code should migrate well in the majority of cases.
There may be an issue where a service and an equipment failure have not been well enough
defined in the historical data.

STARTS: when the rig starts moving to the location where it will be "parked". ENDS: when the rig starts
moving away from the location it was "parked". This will normally be at the start of a "Preparation" phase or
a "Re-entry" phase. COMMENTS: this phase should be used for platform rigs (including jack up rigs
RIG PARKED operating over a platform/jacket) when the rig is parked (i.e. not operating on a well) and Daily Operations RP Rig Parked
reports are still being sent in. This includes when the rig is being used to carry out extraordinary operations
that cannot be classified as work on a well (e.g. doing work on platform deep well pumps for Engineering
Department). The phase typically should be allocated to a utility well.

STARTS: commencement of xmas tree removal – unlocking connector, clamp or flange. ENDS: when
ready to prepare the rig floor to run completion equipment, or the hole is clear and ready to commence
abandon hole section. COMMENTS: this phase includes all activities required to ensure that the hole is in
a condition to move on to the re-completion steps or to return to drilling for sidetrack operations. It includes
removal of xmas tree & installation of BOP’s, removal of tubing, packers, fish or any other debris that may
REPAIR/SPECIFIC WELL have been left in the hole from previous operations. It also includes any clean up activities required prior to Repairs / Specific Well
RQ
REQUIREMENTS preparation for completion. A convenient end point is often the removal of the wearbushing, however, this Requirements
should be used with a degree of judgement. In the case of a sidetrack an alternative end point is when
ready to run a casing cutting string. When moving from the ‘Clear Hole’ step to sidetrack activity, this is
regarded as moving back into the drilling mode. Any activity post ‘Clear Hole’ but prior to sidetrack is
‘preparation’ in the drilling phase. Sidetrack starts after cutting through the casing when new formation is
drilled.

STARTS: if the well is not dead (e.g. having been production tested) it commences at the start of well
killing operations. Otherwise it commences at the start of operations specifically for well suspension.
ENDS: at commencement of rig move to next location. COMMENTS: at any moment during the re-
SUSPENSION completion of a well it can be decided to suspend it. The phase covers any activities carried out by a rig SU Suspension
specifically to suspend a well before moving off it. By definition a "Suspension" phase is always followed
by a "Re-entry" phase. This phase would be used for any operations associated with weather induced
suspensions; e.g. hurricanes.

STARTS: commencement of well intervention activities i.e. running w/line, CT or pumping fluid. ENDS:
tubing hanger plugs have been installed, all barrier tests are complete and accepted and the xmas tree is
SECURE WELL ready to be pulled. COMMENTS: this is a highly variable activity and involves all activities required to SW Secure Well
make the well safe enough to remove the Xmas Tree. This will include establishing two satisfactorily test
barriers.

STARTS: when the tree cap is removed. ENDS: when the "Instatement" phase starts, or when the well is
handed over to the Production Operations Department to serve its planned purpose. COMMENTS: should
be used for well servicing/wireline & coiled tubing work carried out after the well has been perforated,
tested and handed over to the Production Operations Department. Examples of operations in this phase
WELL MAINTENANCE WM Well Maintenance
are stimulation, fracturing, producing the well to clean the well from frac-sand, running tools for production
logging, setting gauges, changing out ball valves, CTU milling operations, TTBP's, TDT's, sand clean outs,

motor, turbine. Includes: Connection times, routine flow
checking after a drilling break, drill pilot hole as part of drill/drive

DRR Drill with Rotary / Top drive Making progress through formation and/or cement in rotary
mode i.e. driving-the drill string at surface with rotary table or top
drive. Includes: connection/MWD survey times, routine flow
checking after a drilling break, drill pilot hole as part of drill/drive
conductor. N.B. In many cases assemblies are used that
function both in "rotary mode" and in "down hole motor" mode,
e.g. assemblies with both turbines and positive displacement
motor
These modes of use can be alternated within one run, e.g. three
singles rotary, one single steering using the down hole motor
only. If that is the case than this should be reflected in the way
the activities are coded. It is possible to record every change in
mode, but in most cases this will be impractical. Instead, one
could divide the time during which the drilling was done
alternating from one mode to the other into two parts, coding
one part with DRR and the other with DRM. Example When
drilling for 10 hours alternating between "rotary" and “down hole
motor" mode, where the “down hole motor" mode takes up 40%
of the time, this could be reported as an activity of 4 hours
coded with DRR and 6 hours coded with DRM. If the codes are
used in this way, meaningful statistics for the performance
analysis of the well can be compiled.

DRS Drill with Rotary Steerable Making progress through formation and/or cement making use
of a rotary steerable assembly. Includes: connection times,
routine flow checking after a drilling break, drill pilot hole as part
of drill/drive conductor, CT drilling operations. N.B. See the
