AFFIDAVIT OF LOSS

I, NIKKO L. RESURRECCION, Filipino, of legal age, and residing at #144

Florencia St., 12th Ave., Gracepark, Brgy. 68, Caloocan City, after having duly sworn in
accordance with the law, do hereby depose and state:

1. That I was a holder of a HIGH SCHOOL DIPLOMA issued by the Digos City
National Highschool, located at Rizal Avenue, Digos Davao Del Sur City;

2. That sometime on the year of 2014, when I was previously residing in

Addition Hills which was located at Block 38, Luya St., Mandaluyong City, the houses
in Addition Hills including my house were burned. Hence, my HIGH SCHOOL
DIPLOMA was also burned.

3. That I have exerted diligent effort to locate the same but only prove futile and
vain. Hence, I already considered my HIGH SCHOOL DIPLOMA beyond recovery;

4. That I am executing the foregoing Affidavit of Loss in order to attest to the

fact of loss of said HIGH SCHOOL DIPLOMA, for whatever legal purpose it may serve,
particularly in support of my application for its replacement or duplicate with my
HIGH SCHOOL DIPLOMA.

AFFIANT FURTHER SAYETH NAUGHT.

IN WITNESS HEREOF, I have hereunto affixed my signature this 24th day of
