1. All CCs/ACCs will conduct this price survey on the 10th of every month in every Constituency. If
10th is a weekend or a closed holiday, fill this out on the very next day. The form Must be filled
only on the specified day.
2. CCs/ACC will send this form immediately to the Program Managers.
3. Program Manager Must send the form FAFEN on or before 15th of every month.
4. Official Price List Must BE acquired from the Magistrate’s Office.
