Health Status of Each Family Member

a. Immunization Status of Family Members

 The children have complete immunization.

b. Healthy Lifestyle Practice

 They often include vegetables in their meals though it is healthy but


their water for drinking is not potable.

c. Use of Protective Measures

 They use mosquito net or mosquito killer during bedtime.

d. Presence of Accident Hazards

 Stairs having high elevation of stairways.

e. Food Storage

 Their foods are being kept in their refrigerator.

f. Cooking Facilities

 They use woods in cooking their foods.

g. Water Supply

 The family uses the water from the rain or the water from the deep
well in drinking,washing their clothes or in kitchen utensils.
h. Toilet facility

 The family has no personal toilet. They just used the toilet donated
by our school SMC.

i. Garbage Disposal

 They disposed their garbage through segregating and burning.

j. Drainage System

 The family has no drainage system because there is no canal


located near n their house.

k. Lighting Facilities

 In terms of their lighting facility, they used bulb since they have
electricity.

1. Unsafe Water Supply

➔ Deep well and rain as a source of water for drinking.


➔ Uses unsafe water for drinking without sterilizing it or doing
other methods so that it will become potable.

2. Accident Hazard

➔ Stairs having far elevation of stairways.

3. Toilet Facility

➔ Absence of personal toilet facility.


➔ Using common toilet.

4. Inadequate living space

➔ House having Inadequate space that may cause of fast


spreading of microorganism.

A. HEALTH THREAT
1. Unsafe Water Supply

 Inability to provide a safe water supply in able to maintain health


and personal adequate sanitation due to:

✔ Lack of knowledge regarding the importance of having safe


water.
✔ Lack of sources of potable water.

2. Accident Hazard (High elevation of stairways)

 Inability to make decision with respect to taking appropriate health


action due to:

✔ Unawareness of the problem

3. Absence of Personal Toilet

 Inability to to make decision in taking appropriate health action due


to:

Lack of knowledge regarding the importance of having their


personal toilet.

4. Inadequate Living Space

 Inability to make decision in able to provide adequate living space


due to:

➢ Unable to see the benefits they can get in having adequate living
space.
