1. INVESTIGATION

i) Process of Investigation

Sutracom Sein Bhd was called in by JKR to come up with remedial work proposal to the
failed slope. Sulracom Sdn Bhd had carried out a site inspection on (elate) to investigate the
extent of the current earth slope failure. The extent of the affected earth slope is assessed by
visual inspection on 29
January 2018.

From site observation, the current failed slope condition is serrous


progressive failure may occur especially after heavy rain.

2. EXISTING SITE CONDITION

i) Topograhpy

Tho current earth utopc face is moslly vegetoted wrth grass, shrubs, durtan trees and other
fruit tree except at Ille slope failure area where tho vegetation has been washed down the slope
and therefore leaves behind a barren earth slope.

ii) Drainage

There is no berm drain and subsoil drains observed on site.

iii) Existing Stabilization Works

There is no stabilization work done on site

J. IMMEDIATE WORKS

Temporary measures had been done on the site, such as laying plastic canvas to cover the
failed areas at this barren slope surface. This is to prevent further water intrusion into the
failed area that may trigger bigger slope failure.

New Jersey Barrier (NJB) and Sand bags had also been placed al the edge of pavement
to prevent water flow to downslope failure area.
4. SUBSURFACE CONDITION

i) Subsoil

The slope is comprised of SILTY soil material with traces of sand and grnvel There is NO Soil
Investigation (SI) done on this site to date after the stope failure occurs.

ii) Ground Water Profile

There is no piozometer installed on site to indicate the ground water table.

iii) Existing Utilities I Services a Electric cublu on electric pipe b. A Pipe Water Uno
c. A New ducting pipe for Telekom cable

5. ST ABILITY OF SITE

i) Failure Description

The reported failure leaves behind a steep barren expose surface, which are highly prone lo
erosion, and surface water seepage that may trigger bigger problem.

ii) Likely Causes

Based on site observation, the present distress slopes were probably initiated by a combination of
the following factors.

a) Current heavy rainfall.


b) Water Seepage from the top.
c) Deterioration of slope material.

iii) Risk Assesment

a. In term of risk, the failure slope poses Higl1 Threat to the road users and also to the existing
electrical pylon and sate

6. METHOD OF IMPLEMENTATION
a. GUNITING
Guniting is one of method implemented for surface protection syslem. Guniting was
proposed in combinalion of the slope protection measures at the site Gun1ting is the process
of spraying dry mix cemenl/sand mortar pneumatically at high velocity on soil surface of slope.

b. SOIL NAILING WORKS

i. Preparation Of Surface
1. Slope to receive soils nails shall be excavated ancl trimmed lo grade.
2. [xcavalion and trimming shnll be limited to maximum 3m
height to allow for 2 rows of soils nails lo be drilled and grouted
hefore procAnfling to the nP.xl 2 rows
3. Set out the positions of soil nails accordinq to ctrnwinn

ii. Fabrication or T25 Rebar Reinforcement


1. High T ensilo steel bar T?S wrth single corrosion protecncn shall be used. Length or the
1Ja1 shall be si11gle length as specified. Bar length exceeding 12rn shall be joined with
MOMENl steel Coupler.
2. l hread1ny al one end of steel bar shall 00 willl th,eacling
machine. Thread length
3. shall be 100mm Thread another end for joining of robar exceedrng 12m
4. PVC centralisers shall be installed at spacing of 2.5m
throughout Hie lenglh of tho high tensile T25 bar.
iii. Drilling Of Borehole
1. Hydraulic Rotary Drilling Machine with drag bi!IDTH down the hammer shall be use
2. Position machine at the required inclination and drill 125mrn
diameter boreholes lo lhe depth required.
3. Flushing of culling shall be with High Pressure Air
Compressor.
4. Should drilling encounter rock, a Down-The-Hote-Harumer shall be used.
5. Where working platform is not available, temporary staging
shall be erected for silling of portable drill rig.
6. Drilling recorcl shall be kept each soil nail. iv. Grouting Of Borehole
1. Upon completion of Drilling, insert the fabricated steel bar into
the drilled hole.grout tube will lower together wilh the steel reinforcement to the bottom of the
borehole.
2. Prepare neat cement groul of water cement ratio Grade 30 (G30) with a grout mixer.
Pumping grout through the inner of the 25mm diameter grout tube until clean grout is
discharges from the top of borehole. Drilled holes left more than z-thrs.shalt be cleaned
before lowering steel bar.

3. Install galvanised steel bearing plate of 200mm x 200rnm x
20mm thick and lock the nail head with washer and two numbers of galvanised nuts after
completion of gurnting works.
4. Construction the nail head cappinq as per drawing.

c. SLOPE STABILIZATION
1. Slope stabilizalion measures are usually implemented together with methods 10 control
drainage and seepage. Horizontal drain has been proposed as one of the subsurface drainage
measures in this project

d. HORIZONTAL DRAIN
I. Preparation Of Surface
1. Slope to receive horizontal drain shall be excavated and trimmed to the required
grade.
2. SP.I out the posi1ions of honzonlal drain accordmq to drawinq.
ii. Fabrication Of Perforated PVC Pipe
1. PVC pipe Class 'D' shall be pre cut to the requned slotted as per drawing and speoucanon
wilh 2 slotted perforations for top section of PVC pipe at 30mm centres throughoul length of
except Ior 75001111 rrom outlet point where no perforation tequirod.
2. Tho pro cut slotted perforuled PVC pipe shall be wrap with one
tayer filter Gectextite. The fllter qectexnlc shaU be NON WOVER Geotextile Po1yfelt TS30
or. technically equivalent
iii. Drilling Of Borehole
