Medical Insurance
All full time employees are eligible to participate in the Orchard Software health insurance plan
immediately upon hire.
The medical insurance plan provides comprehensive coverage for a wide range of medical services,
including but not limited to, physician office visits, maternity care, hospitalization, diagnostic testing,
preventive and well child care, and emergency and urgent care services. The medical insurance plan
also provides prescription drug coverage. Health insurance is provided through UMR, a United
Healthcare Company, using the Choice Plus provider network.
Eligible employees have a choice of two health insurance plan options. The Regular Plan features
lower out-of-pocket costs when utilized. The High Deductible plan features higher out-of-pocket
costs but lower employee premiums. The benefits and exclusions are exactly the same between
both plans.

LAB SERVICES
The lab services program allows you to obtain laboratory-testing services at no cost*. You will need to
search www.umr.com for in-network facilities — you pay no deductibles, no copays and no
coinsurance. This benefit applies to Non-Hospital Facilities and routine lab services only.*

MRI/CT/PET*
To obtain 100% MRI/CT/PET benefits, search www.umr.com for in-network facilities. This benefit

Dental Insurance
The dental insurance plan provides coverage for preventive, basic restorative, major restorative and
orthodontic care. Dental insurance is provided by Guardian. No dental ID is required but you may
print a reference card by visiting www.guardiananytime.com. You can locate a dentist in the
Guardian network and receive the benefits below by also visiting the Guardian Anytime website.

The vision insurance plan provides coverage for a full range of vision services, including eye
examinations, glasses and contact lenses, and laser vision correction. Vision insurance is provided
by VSP. To find an eligible VSP provider, log on to www.vsp.com or call (800) 877-7195. There are
no out of network benefits. If you do not visit a VSP approved provider, your out-of-pocket costs are
100%. There is no vision card required, however, you may visit www.vsp.com and print a vision
reference card.

Smart Ways to Save Money
Use Generic Prescriptions
Did you know that the average brand-name prescription drug costs approximately $85 more than the average
generic? If you switched just one prescription from a brand-name drug to a generic, you could save more than
$1,000 a year. Have a conversation with your physician to see if your brand-name prescription has a generic
equivalent or if there is a generic for a different drug in the same class in order to save money on your
prescriptions.
Is There an Alternative?
Get a second opinion for potentially expensive procedures—ask if alternatives (new medications, nonsurgical
procedures, therapies, etc.) are available. Also, call Teladoc at (877)362-8746 to talk with a doctor—it could
save you a trip to the emergency room.

Flexible Spending Accounts (FSA)
All full time employees are eligible to participate in the Flexible Spending Account program
immediately upon hire. You must re-enroll for this benefit each year during Open Enrollment.
This program lets you pay for certain IRS-approved medical expenses not covered by the insurance
plan with pre-tax dollars.
Budget Carefully – At the end of the year, you will be allowed to carryover up to $500 of unused
health FSA funds into the following plan year. Any funds in excess of $500 will be forfeited.
The Dependent Care FSA lets you use pre-tax dollars towards qualified dependent care such as
caring for children under the age of 13 or caring for elders. The annual maximum amount you may
contribute to the Dependent Care FSA is $5,000 (or $2,500 if married and filing separately) per
calendar year.
Unlike the health care FSA, dependent care FSA's are not "pre-funded"; employees will only receive
reimbursement for the amount deposited into the account as of the reimbursement date.
IRS regulations do NOT allow reimbursements for services that have not yet been provided, so even
if you pay in advance for your expenses, you can only claim service periods that have already
occurred. For example, if you are required to pay for all of January's child care expenses on January
1; you cannot claim the entire month's expense until February. Dependent Care funds do not carry
over.

Disability Insurance
All full time employees are eligible to participate in the Disability Insurance program the first of the
month after thirty days of employment. Disability, Life, Accident, and Critical Illness insurance is
provided by Guardian.
Orchard Software offers short term disability benefits to full-time employees. In the event you
become disabled from a non-work-related injury or sickness, short term disability benefits are
available for up to 90 days after a 7 day elimination period. Pre—existing conditions may apply. Late
enrollees will be required to submit an Evidence of Insurability (EOI) form.
Orchard Software provides full-time employees with long-term disability income benefits, and pays
the full cost of this coverage. In the event you become disabled from a non-work-related injury or
sickness for longer than 90 days, disability income benefits are provided as a source of income.

Accident Insurance
All full time employees are eligible to participate in the Accident Insurance program the first of the
month after thirty days of employment.
Accident insurance is designed to help you meet the out-of-pocket expenses and extra bills that can
follow an accidental injury, whether minor or catastrophic. Benefit payments may be made for
emergency room care, physician office visits and physical therapy. In major accidents, benefits for
catastrophic accident and accidental death may be payable. The accident base plan is guaranteed
issue, so no health questions are required.

Critical Illness Insurance
All full time employees are eligible to participate in the Critical Illness Insurance program the first of
the month after thirty days of employment.
Specified Critical Illness insurance is designed to help you offset the financial effects of a catastrophic
illness with a lump sum benefit (up to $20,000) if you are diagnosed with the first occurrence of a
covered specified critical illness. Covered conditions are heart attack, stroke, major organ transplant,
permanent paralysis, end-stage renal (kidney) failure, coronary artery bypass surgery, cancer, and
carcinoma.
Newly eligible employees electing this benefit in their new hire enrollment window can elect up to
$10,000 employee and $5,000 spouse/children with no medical questions required. Any amounts
over these Guaranteed Issue amounts require evidence of insurability forms to be completed.

Marriage
Notify Orchard within 30 days of the event date in order to add or change coverage.
Child’s Birth/Adoption
Notify Orchard within 30 days of the birth or official adoption date to begin new dependent coverage.
Divorce
Notify Orchard of your change in marital status and any updated beneficiary information within 30
days.
Medicare Eligible
Medicare doesn’t change eligibility for your group health plan if you’re an active employee, but may
provide additional medical options. Contact HR for details.
Vacation/Business Travel
Remember to carry your medical ID card with you.
Address Change
Log in to your Paycom account and select My Information>Address and Contact Information to
update your address.
IMPORTANT
If you do not enroll within your eligibility window, you can NOT make
changes to your coverage until the next open enrollment period.

Special Enrollment Notice
If you are declining enrollment for yourself or your dependents (including your spouse)
because of other health insurance or group health plan coverage, you may be able to
enroll yourself and your dependents in this plan if you or your dependents lose eligibility
for that other coverage (or if the employer stops contributing toward your or your
dependents’ other coverage). However, you must request enrollment within 30 days
after you or your dependents’ other coverage ends (or after the employer stops
contributing toward the other coverage).
Finally, you and/or your dependents may have special enrollment rights if coverage is
lost under Medicaid or a State health insurance (“SCHIP”) program, or when you and/or
your dependents gain eligibility for state premium assistance. You have 60 days from
the occurrence of one of these events to notify the company and enroll in the plan.

Glossary
401(k) Plan
A 401(k) is a retirement investment account that enables eligible employees to make contributions
from their paycheck on a pre-tax basis.
Beneficiary
A person or entity named in a financial contract as the inheritor of income or property when the owner
dies.
COBRA
Consolidated Omnibus Budget Reconciliation Act -- a federal law that allows employees to continue
healthcare coverage under a company’s group plan, based on a “qualifying event” such as
termination of employment or loss of dependent status.
Co-insurance
Co-insurance is a cost-sharing feature of health insurance coverage that requires the insured to pay a
percentage of the covered services. Generally this is based on the allowed, or contracted, amount
that the insurance covers. For example - insurance pays 80% of the allowed, and the member is
responsible for the 20% balance.
Coordination of benefits
Referred to as "COB" this is the practice of coordinating the benefits between two insurance
companies so that the benefits are maximized, but not duplicated. There are industry standards on
which insurer should be “primary” (pays first) or “secondary” (pays second).
Co-pay
Co-pay is a cost-sharing feature of health insurance coverage that requires the insured to pay a fixed
amount for specific services, such as an office visit co-pay, or pharmacy co-pay.
Deductible
The amount that must be satisfied before the insurance plan provides payment. The amount that is
put toward your deductible is calculated based on the health care provider’s actual charge or the
allowed charge, whichever is less. The member is responsible for paying the charges applied to the
deductible.
EAP
Employee Assistance Program is designed to help employees manage a variety of personal issues.
ERISA
Employee Retirement Income Security Act -- a 1974 federal statute which governs self-funded
employer sponsored health plans. ERISA groups are generally exempt from state mandated health
benefits, but are subject to many federal mandates.

Flexible spending account (FSA)
Flexible spending accounts allow an employee to contribute a predetermined amount of pre-tax pay
to an account that can be used to pay for qualifying expenses. Depending on the type of FSA, those
expenses might include childcare or medical costs that aren't covered by health insurance. At the end
of the plan year, any unspent money in a FSA may be forfeited.
HIPAA
Health Insurance Portability & Accountability Act of 1996. The intent of HIPAA is to improve access
and “portability” of healthcare coverage. It also addresses standardized code sets for the
transmission of health insurance information and sets the standard for confidentiality by regulating the
privacy protection of patients’ identity and medical information.
In-network provider
An in-network provider is any health care provider (physician, hospital, etc.) that belongs to a health
plan's network. Using an in-network provider will usually cost members less in copayments or co-
insurance.
Out-of-network provider
An out-of-network provider is any health care provider that does not belong to our provider networks.
Members can use their benefits for out-of-network expenses, but miss out on in-network discounts.
PPO
Preferred Provider Organization - a type of health plan with a network of providers who are
"preferred", that is, a member can visit these preferred in-network physicians and does not have to
choose a primary care physician. Generally members can also visit non-preferred or out-of-network
providers, but may have to pay a higher fee or copayment.
Referral
If a PCP determines that an individual requires specialized care, the PCP may "refer" that person to
an appropriate specialist.
Roth 401(k)
A Roth 401(k) is retirement investment account that enables eligible employees to make contributions
from their paycheck on a post-tax basis.
Vested
When a benefit is vested, an employee has a right to the income from that benefit. For example,
when an employee is vested in a retirement plan, he or she rightfully owns the funds that Orchard
Software has invested in the account in his or her name and can take possession of those funds
under specified conditions.
Workers’ Compensation
An insurance policy maintained by employers to cover health care costs in the event of a work related
injury.
