1. Write the types of industries that required safety and health officer, according to
SHO Order.

2. Explain the requirements as outlined in USECCH,2000 for employer to ensure

safety and health matters of their employee and other person under their
responsibility.

3. Delicious Pastries Sdn Bhd, has been directed by the Director General of
Department of Occupational Safety and Health to have safety and health
committee [SHC] at their workplace.

a. Establish the SHC [in term number of members and the composition] for
Delicious Pastries Sdn Bhd which has total number of worker of 250.

b. Explain the reason for your answer in 3a according to statutory law(s) (Act
/ Regulation/ Order/ Code of Practices/ Guidelines; whichever applicable).

4. During the routine inspection of DOSH officer to a petrochemical plant of 250 staff,
Excell Sdn Bhd; the officer had instructed a new safety and health committee
(SHC) to be reestablished. If you were the DOSH officer, point out the
UNCOMPLIANCES according to regulation {state the reason and name of
regulation that has been referred}. Details of the present SHC at Excell Sdn Bhd
as listed in Table 2.

4. According to Occupational Safety and Health Act, 1994 an organization need to

prepare their safety and health policy if they have 5 or more workers in the
organization. The organization also needs to establish their safety and health
committee if there are 40 or more workers in their employer list.
i) Define safety and health policy.
ii) Explain the functions of Safety and Health committee.

5. Write down FIVE (5) elements of successful OSHMS according to OHSAS

18001.

6. Discuss the pathway/method of becoming competent Safety and Health Officer.
